超声引导技术在深静脉置管的应用 被引量:11

摘要 目的对超声引导技术用于深静脉置管应用的临床评价。方法预行深静脉静脉置管治疗的患者4 500例,按随机数字表法分成3组,每组1 500例。A组超声动态引导下穿刺,B组超声定位后穿刺,C组(传统方法)盲探穿刺。记录各组穿刺成功例数、插管所需次数、并发症等。结果 C组更换穿刺位置33例,15例穿刺失败放弃置管;A、B、C组一次穿刺成功分别为1 493(99.5%)例、1 471(98.1%)例和1 109(73.9%)例,穿刺成功次数分别为1 512次、1 545次和2 597次,一次成功置管及所需次数C组明显多于A、B组(P<0.05);C组有2例患者出现气胸,26例颈部血肿,7例误入动脉,3例出现神经损伤。结论超声引导下行深静脉置管提高了穿刺成功率,能减少并发症的发生,有较高的临床应用价值。
