彩色多普勒超声和多层螺旋CT对甲状腺结节显示能力的比较分析

Investigation of the Display Abilities of Color Doppler Ultrasound and Multi-slice Computed Tomography on Thyroid Nodules: A Comparative Study

目的探讨彩色多普勒超声(CDUS)与多层螺旋CT(MSCT)对甲状腺结节的显示能力。方法回顾性分析57例经手术病理确诊的甲状腺结节患者的CDUS、MSCT及临床资料。分别统计两种检查方法对甲状腺结节检出率的高低及对包膜、钙化及其内部密度均匀性的显示能力。结果57例甲状腺结节患者手术病理共检出113枚结节,113枚结节均可见包膜,瘤周或瘤内有钙化灶43枚,无钙化灶的70枚瘤体内密度均匀32枚,不均匀38枚。术前CDUS、MSCT分别检出97枚(85.8%)、89枚(78.8%)结节,有包膜的结节分别为88枚(90.7%)、73枚(82.0%),有钙化灶结节分别为43枚、43枚(100.0%),结节内密度不均匀分别为37枚(68.5%)、21枚(45.6%)。术前CDUS与MSCT对甲状腺结节的检出率及对包膜及钙化的显示能力差异无统计学意义(P>0.05),但CDUS对瘤内密度均匀性显示能力优于MSCT(P<0.05)。结论 CDUS与MSCT对甲状腺结节均具有较高的检出率及显示能力,但CDUS对包膜、钙化及瘤内密度均匀性显示能力方面优于MSCT,CDUS为首选的检查方法。

Objective To investigate the display abilities of color Doppler ultrasound （ CDUS ） and multislice CT （MSCT） on thyroid nodule.Methods CDUS,MSCT and clinical data of 57 patients with thy-roid nodules confirmed by operation pathology were retrospectively analyzed .Detection rates of two methods on thyroid nodule and display abilities of nodule envelope,calcification and internal density homogeneity of the two methods were to statistically analyzed respectively .Results There were 113 nodules detected by operation pathology in 57 patients with thyroid nodules.113 nodules had visible envelope.There were 43 nod-ules which had calcifications surrounding or inside the nodules .For 70 nodules with no calcification , there were 32 with internal density homogeneity and 38 with no internal density homogeneity .The nodules detection rates of CDUS and MSCT before the operation were 85.8%（97） and 78.8%（89） respectively.Display abil-ities of the nodules with envelope were 90.7%（88） and 82.0%（73） respectively.Display abilities of the nodules with calcification lesion were 100.0%（43） and 100.0%（43） respectively.Display abilities of nod-ule internal density uneven were 68.5 %（37） and 45.6% （21）.There were no difference with regard to detection rates of CDUS and MSCT on thyroid nodule and display abilities on envelope and calcification before the operation （P〉0.05）.But CDUS was better than MSCT with regard to the display ability of nodule internal density homogeneity（P〈0.05）.Conclusion Both CDUS and MSCT have high detection rates and display abilities on thyroid nodule,but CDUS is a little higher than MSCT in display abilities of the envelope ,calcifica-tions and nodule internal density homogeneity,therefore CDUS is the preferred examination method.
