妊娠期并发肿瘤对孕妇分娩结局和新生儿的影响

Affect of concurrent tumors in pregnancy outcomes for pregnant women and newborn

目的探讨妊娠期并发肿瘤对孕妇分娩和新生儿健康的影响。方法选取2003年1月至2014年12月间广东省广州市妇女儿童医疗中心收治的妊娠期合并肿瘤的41例孕妇作为观察组,另选50例同期分娩但无肿瘤合并症的孕妇作为对照组。对观察组孕妇的肿瘤类型、两组患者的分娩方式、分娩出血量以及妊娠并发症和新生儿基本情况进行回顾性分析。结果观察组孕妇肿瘤发生情况以宫颈癌最高,发生率为21.9%,卵巢癌和白血病次之,均为14.6%。与对照组相比,观察组孕妇发生先兆流产的发生率明显偏高(P<0.05);观察组胎膜早破发生率明显偏高(P<0.05);观察组阴道分娩发生率明显偏低(P<0.05);观察组胎儿发生宫内窘迫的发生率显著高于对照组(P<0.05),新生儿窒息发生率也明显偏高(P<0.05),低体重出现率也明显高于对照组(P<0.05)。结论妊娠期并发肿瘤会增加妊娠并发症、剖宫产发生率,造成新生儿窒息、胎儿窘迫等不良反应,临床上应予以更多关注。

Objective To evaluate the effect of pregnancy combined with tumor on childbirth and health of newborn. Methods From January 2003 to December 2014 in Guangzhou Women and Children＇s Medical Center, 41 pregnant women combined with tumor were enrolled in the study as the observation group, and 50 pregnant women without tumor in the same period were served as control group. Tumor types, mode of delivery, the amount of bleeding and pregnancy complications and newborn basic information were recorded and analyzed retrospectively. Results In the observation group, the incidence of cervical cancer was highest （21.9%）, and the following was ovarian cancer and leukemia （ both were 14.6% ）. The incidence of threatened abortion in the observation group was significantly higher than that of the control group （x2 = 6. 213,P 〈 0. 05）. The incidence of premature rupture of membranes in the observation group was also significantly higher than that of the control group （x2 = 8. 221 ,P 〈 0. 05 ）. The incidence of vagi- nal delivery in the observation group was significantly lower than that of the control group （ x2 = 5. 239, P 〈 0. 05 ）. The incidence of fetal distress and asphyxia neonatorum in the observation group was significant- ly higher than that of the control group （P 〈 0. 05）. Conclusions Pregnancy combined with cancer will in- crease pregnancy complications, the incidence of cesarean section, and can cause asphyxia, fetal distress and other adverse reactions, so more attention should be paid.
