宫颈上皮内瘤变细胞中Ki67的表达及与HPV感染相关性研究

Study on correlation between Ki67 expression in cervical intraepithelial neoplasia cells with HPV infection

目的研究Ki67在宫颈上皮内瘤变（CIN）细胞涂片中的表达情况及其与人乳头瘤病毒（HPV）感染的相关性,探讨Ki67作为宫颈病变筛查的一种新的有效指标的可行性。方法采用免疫组织化学检测Ki67在138例不同程度宫颈病变细胞涂片中的表达,同时取40例宫颈炎患者作对照,并与病理学及HPV分型检测结果进行比较。结果 Ki67在宫颈炎、CINⅠ、CINⅡ~Ⅲ组中的阳性率分别为7.5%（3/40）、60%（30/50）、87.5%（42/48）,各组差异有统计学意义（P〈0.01）。HPV在宫颈炎、CINⅠ、CINⅡ~Ⅲ组中的阳性率分别为20%（8/40）、66%（33/50）、91.7%（44/48）,各组差异有统计学意义（P〈0.01）。Ki67的表达与HPV感染在CINⅠ组、CINⅡ~Ⅲ组中呈正相关,相关系数在CINⅠ组、CINⅡ~Ⅲ组分别为0.63、0.66（P〈0.01）。结论 HPV的感染与宫颈鳞癌的发生密切相关,并与进展有关。检测Ki-67可作为筛查宫颈病变、判断预后的有效指标。

Objective To study the expression level of Ki67 in cell smear of cervical intraepithelial neoplasia（CIN）and its relationship with human papilloma virus（HPV）infection,and to investigate the feasibility of Ki67 as a new effective indicator in screening of cervical lesion.Methods The expression of Ki67 was detected by immunocytochemical method in 138 cases of different grade cervical lesions,at the same time,40 cases of cervicitis were chosen as control and the detection results were compared with the pathology and HPV typing detection results.ResultsThe positive rate of Ki-67 in cell smear of cervicitis,CINⅠ and CINⅡ-Ⅲ groups were 7.5%（3/40）,60%（30/50）,87.5%（42/48）respectively,the differences were statistically significant（P〈0.01）.The positive rate of HPV in cervicitis,CINⅠ and CIN Ⅱ-Ⅲ groups were 20%（8/40）,66%（33/50）and 91.7%（44/48）respectively,the differences were statistically significant（P〈0.01）.The expression of Ki67 in CINⅠand CIN Ⅱ-Ⅲ groups was positively correlaed with HPV infection.The correlation coefficients were 0.63 and 0.66respectively（P〈0.01）.Conclusion HPV infection is closely related with the cervical squamous cell carcinoma occurrence and its progress.The detection of Ki67 could be taken as an effective indicator for screening of cervical lesion and judging prognosis.
