关节镜辅助下治疗胫骨平台劈裂塌陷骨折20例

Arthroscopy-assisted treatment for tibial plateau splitting and collapsed fracture: A report of 20 cases

目的 探讨关节镜下治疗胫骨平台劈裂塌陷骨折的临床疗效。方法 20例胫骨平台劈裂塌陷骨折患者,根据Schatzkerfen分型,Ⅱ型8例,Ⅲ型9例,Ⅳ型3例,采用关节镜监视下复位固定,术后随访11～14个月,按HSS膝功能评定标准进行疗效评定。结果 20例患者术后均无感染、深静脉血栓、切口愈合不良、骨筋膜间室综合征等并发症发生,HSS评分优良率为90%。结论 关节镜下治疗胫骨平台劈裂塌陷骨折,效果良好,值得临床推广应用。

Objective To investigate the clinical efficacy of arthroscopy-assisted treatment for tibial plateau splitting and collapsed fracture. Methods Twenty patients with tibial plateau splitting and collapsed fracture were enrolled in this study. Of the 20 patients, 8 patients were diagnosed as type Ⅱ, 9 patients as type Ⅲ and 3 patients as type lV according to the Schatzkerfen classification. All of the patients were treated with arthroscope-gnided reduction and internal fixation and were followed up for 11 to 14 months. The efficacy was assessed according to HSS knee function evaluation criteria. Results None of the patients were complicated with infection, deep vein thrombosis, poor wound healing and steofascial compartment syndrome. The excellent and good rate was 90% depending on HSS scores. Conclusions Arthroscope-gnided reduction and internal fixation is an effective approach for tibial plateau splitting and collapsed fracture. It is worthy of clinical application and promotion.
