摘要抗甲状腺药物（anti-thyroid drugs,ATD）包括丙基硫氧嘧啶和甲巯咪唑,其作用机制是抑制甲状腺过氧化物酶,从而抑制甲状腺素的合成。治疗甲亢时,ATD的使用通常分为初始、减量和维持三个阶段,总疗程达到1.5~2年。停用ATD后疾病的缓解率仅为50%左右,且MMI和PTU这两种药物的甲亢缓解率相似。ATD的不良反应较常见,大多数为轻度不良反应,严重不良反应有粒细胞缺乏和肝衰竭。PTU发生严重不良反应的风险显著高于MMI,因此MMI是治疗甲亢的首选药物。妊娠早期应使用PTU而禁用或慎用MMI,在妊娠中晚期可换用MMI,哺乳期女性服用PTU或MMI都是安全的。
