双能X射线骨密度检查发现Fanconi综合征一例

摘要 患者男,56岁,因全身多处骨痛2年,加重2个月就诊.患者2年前出现全身多处骨痛,主要部位为双侧膝关节,近半年出现腰骶部、双侧肋骨疼痛伴活动受限,无红肿晨僵.2个月前疼痛加重,尚能行走.外院腰椎CT示腰椎退行性病变,诊断为双膝关节炎,未予特殊治疗.本院双能X射线骨密度检查示T值:L1~4为-2.8,左侧股骨颈为-2.9,全髋为-2.6;X线检查(图1)示:T12、L1压缩性骨折,双侧胫腓骨密度不均,股骨、骨盆构成骨可疑低密度影,提示严重骨质疏松.99 Tcm-MDP全身骨显像(图2)示多发性椎体及肋骨病变,双侧膝关节病变.患者既往有肝炎、肝硬化病史,2006年开始口服阿德福韦酯10 mg/d、拉米夫定各100 mg/d治疗至今.
