血管内皮生长因子与冠心病患者再发心血管事件的相关性 被引量:7

Correlation between Vascular Endothelial Growth Factorand Recurrent Cardiovasculareventsin Patients with Coronary Heart Disease

摘要 目的:探讨血管内皮生长因子(VEGF)与冠心病(CHD)患者再发心血管事件的相关性及影响因素。方法:120例CHD患者根据VEGF(pg/ml)结果分为3组,VEGF<50组(A组)55例,VEGF50-200组(B组)42例、VEGF>200组(C组)23例,以Gensini评分判断冠状动脉病变范围和程度,用Endo-PAT2000测得的反应性充血指数(RHI)评估患者外周动脉张力。随访6个月,观察再发心绞痛、再住院和死亡等心血管事件的发生率及VEGF的相关影响因素分析。结果:C组6个月后再发心绞痛显著高于其他两组,P<0.05,再发心绞痛患者VEGF水平显著高于未出现心绞痛者(P<0.05),三组再住院和死亡率各组间无统计学意义(P>0.05)。多重线性回归分析显示,VEGF与冠脉狭窄程度、冠脉Gensini评分、TG呈正相关(P均<0.05),与RHI负相关(P<0.05)。结论:C组6个月后再发心绞痛显著增高,VEGF与冠状动脉狭窄的程度、Gensini评分、TG及RHI显著相关,VEGF水平显著增高对于CHD患者预后不利,可作为评估CHD患者预后的一项指标。 Objective: To investigate the vascular endothelial growth factor(VEGF) and coronary heart disease(CHD) in patients with recurrent cardiovascular events correlation and influencing factors. Methods: 120 cases of CHD patients with VEGF(PG / ml) were divided into three groups according to the VEGF 50 groups of(group A) 55 cases, 42 cases of VEGF50-200 group(group B), vascular endothelial growth factor(VEGF) 200 group(group C) in 23 cases, with Gensini score to determine the extent of coronary artery disease and degree, Endo-PAT2000 measurement of reactive hyperaemia index(RHI) assessment in patients with peripheral arterial tension. Patients were followed up for 6 months to observe the recurrent angina and readmission and death and other cardiovascular events occurred rate of VEGF and influence factors analysis. Results: Group C at 6 months after the recurrence of angina was significantly higher than those in the other two groups, P〈0.05, and VEGF levels in patients with angina pectoris were significantly higher than those without(P〈0.05) angina pectoris, 3 group re hospitalization and mortality in each group no statistical significance(P〉0.05). Multiple linear regression analysis showed that, VEGF and the degree of coronary artery stenosis, Gensini score, TG were positively related(P〈0.05), with RHI negative correlation(P〈0.05). Conclusion: Group C at 6 months after the recurrence of angina was significantly higher, were significantly related to vascular endothelial growth factor(VEGF) and coronary artery stenosis degree, Gensini score, TG and RHI, VEGF levels were significantly higher in bad for the prognosis of patients with CHD, as an index of evaluating the prognosis of patients with CHD.
