摘要
目的分析耳声发射未通过而ABR反应阈正常的婴幼儿听力检测结果,探讨合适的中耳功能诊断方法。方法选取经过听力检测ABR反应阈正常但耳声发射不通过的婴幼儿30例(60耳)作为研究组,另外选取同时期ABR反应阈正常且耳声发射通过的婴幼儿30例(60耳)作为对照组,进行听性脑干反应(ABR),畸变产物耳声发射(DPOAE),1 000 Hz鼓室声导抗等听力学检查。结果研究组60耳中,有53耳ABR波I潜伏期延长;两组间比较,ABR波Ⅰ、Ⅲ、Ⅴ潜伏期差异有统计学意义;1 000 Hz声导抗测试结果显示研究组60耳中52耳均为平坦型。结论中耳功能障碍是导致ABR反应阈正常但耳声发射不通过的主要原因;1 000 Hz鼓室声导抗测试以及ABR波I潜伏期,可作为判断婴幼儿中耳功能正常与否的客观有效的检查指标。
Objective To analysis the results of the hearing test of infants with normal ABR reaction threshold,and investigate the appropriate diagnostic method for middle ear function. Method 30 infants( 60 ears) with normal ABR reaction threshold while OAE refer were selected as the research group,in addition,30 infants( 60 ears) were selected as the control group with normal ABR reaction threshold and OAE pass. Auditory brainstem response( ABR),distortion product otoacoustic emission( DPOAE),1 000 Hz tympanometry and audiological examination were conducted. Results There were 53 I latency of ABR wave prolonged in the 60 ears of the research group; there were significant differences in the latency of I,III and V between the two groups; the anti test results of 1 000 Hz tympanometry and audiological showed that the 52 ears In the 60 ears of the research group were flat. Conclusion The dysfunction of middle ear is the main reason that causes the normal ABR reaction threshold but OAE not pass;1 000 Hz tympanometry test and ABR wave I latency,as judged by the infant middle ear function is normal or not the objective and effective examination index.
