补肾活血化浊汤联合西药治疗慢性再生障碍性贫血随机平行对照研究

Randomized Controlled Study of Bushen Huoxue Huazhuo Decoction Combined with Drug of Chronic Aplastic Anemia

[目的]观察补肾活血化浊汤联合西药治疗慢性再生障碍性贫血疗效。[方法]使用随机平行对照方法,将48例门诊患者按就诊顺序编号方法随机分为两组。对照组24例司坦唑醇,6mg/d,3次/d;环孢菌素A,3mg/kg·d-1,3次/d。治疗组24例补肾活血化浊汤(熟地、何首乌各30g,菟丝子、补骨脂、山慈菇、大贝各15g,枳实10g,丹参30g,三七5g等),1剂/d,水煎150m L,分3次口服;西药治疗同对照组。连续治疗3个月为1疗程。观测临床症状、红细胞、网织红细胞、血红蛋白水平、骨髓增生程度、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈22例,缓解1例,明显进步1例,无效0例,总有效率85.83%。对照组痊愈14例,缓解2例,明显进步2例,无效6例,总有效率66.67%。治疗组疗效优于对照组(P<0.05)。红细胞、网织红细胞、血红蛋白水平两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。骨髓增生程度两组均有降低/升高(P<0.05),治疗组降低/升高优于对照组(P<0.05)。[结论]补肾活血化浊汤联合西药治疗慢性再生障碍性贫血,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the effect of Bushen Huoxue Huazhuo decoction combined with western medicine curative effect in the treatment of chronic aplastic anemia. [Method] Used random parallel control method, 48 outpatients number methodaccording to the visiting sequence were randomly divided into two groups. A control group of 24 cases of stanozolol, 6mg/d, 3 times /d; cyclosporin A3 mg/Kg·d-1,3/d. The treatment group of 24 cases of Bushen Huoxue Huazhuo Decoction（radix rehmanniae, Radix Polygoni Multiflori 30 g respectively, dodder seed, Fructus Psoraleae, mountain arrowhead, Bei 15 g, orange 10 g, Salvia 30 g,three seven 5g, 1/d） agent, water frying 150 m L, points 3 times oral western medicine treatment with the control group. 3 months of continuous treatment for 1 courses. Observation of clinical symptoms, red blood cell, reticulocyte,hemoglobin level, the degree of bone marrow hyperplasia, adverse reaction.Continuous treatment of 2 courses of treatment, curative effect. [Results] 22 cases cured in treatment group, relieved in 1 cases, significantprogress in 1 cases, 0 cases ineffective, the total efficiency of 85.83%. The control group cured 14 cases, relieved in 2 cases, significant progress in 2 cases, 6 cases ineffective, the total efficiency of 66.67%. The efficacy of the treatment group was better than that of the control group（P〈0.05）. Red blood cell, reticulocyte hemoglobin level, two groups were improved（P〈0.05）, the treatment group is better than control group（P〈0.05）. The degree of bone marrow hyperplasia in two groups were decreased / increased（P〈0.05）, the treatment group decreased / increased than that of the control group（P〈0.05）. [Conclusion] Bushen Huoxue Huazhuo decoction combined with western medicine in the treatment of chronic aplastic anemia, thecurative effect is satisfied, no serious adverse reaction, is worth promoting.
