进行性对称性红斑角皮症伴发Pinkus纤维上皮瘤1例

A Case of Symmetrical Progressive Erythema Keratoderma Associated with a Fibroepithelioma of Pinkus

患者女,36岁。双侧手足角化性红斑36年,右侧乳房增生物2年。皮肤科情况:双侧手掌、足底弥漫性、对称性分布角化性红斑,轻度浸润肥厚,皮损波及腕关节和足背,上附有少许片状鳞屑,足底有轻度胼胝样改变。右乳头内侧可见一约黄豆大小的圆形、肤色增生物。右乳头内侧肿物组织病理检查示:真皮浅中层基质间可见与表皮相连的上皮样细胞条索,部分吻合成网状,个别视野可见囊肿形成及毛胚芽结构,间质淡染,可见星状成纤维细胞增生,黏蛋白沉积。诊断:进行性对称性红斑角皮症伴发Pinkus纤维上皮瘤。

A 36-year-ohl female presented with neoplasm on her right breast for 2 yea~ and with bilate keratosis erythe- ma on hands and feet for 36 years. Dermatology examination showed extensive and symmetrical keratosis ery- thema o, bilateral palm and feet that with mild infiltrating and hypertrophy,lesions spread to the wrist and dorsum of the foot,with a little scales. Callosity was seen on the fi^ot. A soybean round and cuticolour neo- plasm couhl be seen on the right breast. The pathological examination of the neoplasm showed in the dermis matrix,epithelioid cell that linked with epidermis, and some epithelioid cell shaped with mesh. Cysts and hair germ structures could be seen. Stellate fibroblasts proliferation and mucin deposition could be seen during the matrix. The diagnosis of progressive erythema keratoderma associated with a fibroepithelioma of Pinkus was made.
