血浆脂蛋白磷脂酶A2、同型半胱氨酸与缺血性脑卒中的研究进展 被引量:3

摘要 脑卒中临床分为出血性和缺血性两大类。TOAST( Trialof ORG10172 in Acute Staroke Treatment)将缺血性脑卒中分为心源性脑栓塞(CE)、大动脉粥样硬化性脑卒中(LAA)、小动脉脑卒中(SAA)、其他原因引发的缺血性脑卒中(SOE)、原因不明的缺血性脑卒中(SUE)[1]。其中LAA是缺血性脑卒中最常见的病理类型,包括大动脉血栓和动脉-动脉间栓塞,主要原因是动脉粥样硬化后斑块形成,节段性狭窄、破溃、斑块脱落到狭窄处,造成管腔阻塞。动脉粥样硬化(ath-erosclerosis,As)是一种全身弥漫性的病变,易累及全身各个大中动脉血管,尤以脑动脉、颈动脉、冠状动脉等动脉明显。近年来,许多研究结果表明,As的发生是由多种炎性因子协同参与的一种疾病,在As中斑块的发生、发展以及破裂等各个环节均有相应的炎性因子参与,所以心血管疾病中的炎症指标作为心血管危险预测因子而引起了医学界广泛的关注。脂蛋白磷脂酶A2(Lp-PLA2)和同型半胱氨酸(Homocysteine,Hcy)作为近几年来发现的与心脑血管疾病相关的两个炎性因子,其生理机制及在心血管疾病中的作用逐渐成为研究的热门。现将与其相关的基本理论和研究进展综述如下。
