PI3K表达与宫颈鳞状细胞癌局部血管生成和浸润转移及预后的关系

Relationship between the expressions of PI3K and regional angiogenesis,invasion,metastasis,and prognosis in cervical squamous cell carcinoma

摘要 目的探讨PI3K在宫颈鳞状细胞癌(SCC)组织的表达及其临床意义。方法采用免疫组化SP法检测PI3K在28例宫颈正常上皮(NCE)、36例宫颈上皮内瘤变(CIN)和68例SCC组织的表达,并检测其中Ki-67标记的增殖指数(PI)和CD34标记的微血管密度(MVD)。结果 PI3K在NCE、CIN及SCC组的阳性表达率分别为25.00%、55.56%及85.29%。PI3K在SCC组的阳性表达率分别显著高于CIN和NCE组(P<0.05)。PI3K在SCC组表达分别与PI及MVD显著正相关。PI3K在SCC组表达分别与FIGO分期、组织学分级及有否盆腔淋巴结转移有关(P<0.05),而与患者年龄、有否脉管浸润、间质浸润深度及生存率无关(P>0.05)。宫颈鳞癌FIGO分期为Ⅱ期、组织学分级为Ⅲ级及有盆腔淋巴结转移者,其PI3K阳性表达率分别显著高于FIGO分期为Ⅰ期、组织学分级在Ⅱ级以内及无盆腔淋巴结转移者(P<0.05)。结论 PI3K过度表达可能上调宫颈鳞癌细胞增殖及间质血管新生,可能促进宫颈鳞癌转移、从而促进其病程进展。 Objective The expressions of PI3 Kin invasive squamous cell carcinoma of cervix and their clinical significance were investigated.Methods The expressions of PI3 K,Ki-67 and CD34protein in 28 cases of normal cervical epithelium(NCE),36 cases of cervical intraepithelial neoplasm(CIN)and 68 cases of squamous cell carcinoma of cervix(SCC)were detected by immunohistochemistry SP method.Results The positive expression rates of PI3 Kin NCE,CIN and SCC were 25.00%,55.56% and 85.29%,respectively.The positive expression rates of PI3 Kincreased remarkably from NCE and CIN to SCC(P〈0.05).The expression of PI3 Kin SCC was significantly positive correlated with PI and MVD.The expression of PI3 Kin SCC was related with FIGO staging,histological grading and pelvic lymph node metastasis(P〈0.05),but was not related with age,depth of stroma infiltration and intravascular infiltration(P〉0.05).In the cases with FIGO stagingⅡ,poorly differentiated tissue and pelvic lymph node metastasis,The positive expression rate of PI3 Ksignificantly higher than those in the cases without them(P〈0.05).Conclusion The over-expression of PI3 Kmay lead to up regulation of tumor angiogenesis and cancer cell proliferation in SCC,and may promote tumor Metastasis and progress of SCC.
