中药方分别辅助腰硬联合阻滞内固定与外支架固定用于复杂性胫骨平台骨折的疗效对比观察

Comparative observation the effect of prescription auxiliary respectively fixation combined spinal epidural and external bracket in complex tibial plateau fractures in complex tibial plateau fractures

目的探讨中药方辅助腰硬联合阻滞内固定与外支架固定用于复杂性胫骨平台骨折的临床疗效差异。方法将220例复杂性胫骨平台骨折患者随机分为试验组和对照组各110例。试验组给予腰硬联合麻醉内固定,对照组行外支架固定术,2组患者均给予中药方辅助治疗和运动锻炼。观察2组手术时间、住院时间和最早负重时间;膝关节功能评分;临床疗效和不良反应发生率。结果试验组患者手术时间、住院时间和最早负重时间均长于对照组,差异均有统计学意义(P<0.05)。试验组患者疼痛评分、膝关节稳定性、运动能力、运动范围和患者自评评分均高于对照组,差异均有统计学意义(P<0.05)。2组影像学评分差异无统计学意义(P>0.05)。试验组总有效率高于对照组,不良反应率低于对照组,差异均有统计学意义(P<0.05)。结论中药方辅助腰硬联合阻滞内固定治疗复杂性胫骨平台骨折虽然所需治疗时间较长,但膝关节功能评分和疗效更佳,值得临床推广应用。

Objective Analysis the differences of clinical effection of tibial plateau fracture by spinal epidural anes -thesiaforfixationandexternalfixationcombinedwithtraditionalChinesemedicine.Methods 220patientswithtibialplateau fractures were randomly divided into test group and control group ,each of 110 cases.The test group was treated by spinal epi-dural anesthesia for fixation ,while the control group was treated by external fixation .2 groups received the prescription assist and exercise.After treated,compared the operation time,the length of stay,first load time,knee function score,clinical efficacy and the incidence of adverse reactions of 2 groups.Results The operative time,length of stay and the first load time of test group was longer than that of control group ,the difference was statistically significant （P〈0.05）. The pain scores,knee sta-bility,athletic ability,range of motion,and patient self-assessment score of test group were higher than that of control group ,the difference was statistically significant（P〈0.05）. The imaging studies score of 2 groups was no statistically significant（P〉0.05）. The total efficiency of test group was higher than that of control group ,adverse reaction rates was lower than that of control group,the difference was statistically significant （P〈0.05）. Conclusion Chinese herbal medicine prescription auxil-iary epidural block fixation in complex tibial plateau fractures has longer treatment time ,and knee function score and efficacy are well,Worthy of clinical application .
