乳腺癌患者癌因性疲乏、应对方式及生活质量的研究

The study of cancer- related fatigue,coping style and quality of life in breast cancer patients

目的调查住院乳腺癌患者的癌因性疲乏、应对方式及生活质量状况,分析三者之间的关系。方法采用癌症疲乏量表、简明应对量表、癌症患者生命质量测定量表对北京市某三级甲等医院205例乳腺癌住院患者进行调查。结果乳腺癌患者癌症疲乏总分(21.96±7.92)分;消极应对得分低于国内常模(P<0.01);生活质量总分为(91.10±20.33)分。多元逐步回归分析发现年龄、居住地、医疗费用负担、目前治疗(内分泌、靶向治疗)、躯体疲乏、情感疲乏、积极应对、消极应对乳腺癌患者生活质量有影响(P<0.05)。结论乳腺癌患者的生活质量处于中等偏低水平,尤其是情感状况和功能状况,减轻疲乏、提高应对水平有助于改善其生活质量状况。

Objective To investigate the cancer - related fatigue, coping styles and quality of life（QOL） in breast cancer patients and analyze the relation- ship among them. Methods The Cancer Fatigue Scale （ CFS ）, Simplified Coping Style Questionnaire （ SCSQ）, Functional Assessment of Cancer Therapy （FACT） were used in 205 breast cancer inpatients in a tertiary hospital of Beijing. Results The average score of cancer fatigue was （21.96 ± 7.92）. Negative coping score was below the domestic norm（ P 〈 0.01 ）. And the quality of life score was （91.10 ± 20.33 ）. Multiple stepwise regression analysis showed that age, place of residence, the burden of medical expenses, the treatment （ endocrinotherapy and targeted therapy） ,body fatigue, emotional fatigue,positive coping and negative coping were influence factors of quality of life （ P 〈 0. 05 ）. Conclusion The quality of life of patients with breast cancer is at the medium to low level, especially the emotional and functional status. Relieving fatigue and improving postive coping may help improve the quality of life of them.
