创伤性细微骨折的MRI影像学研究及其临床应用

摘要 目的分析研究创伤性细微骨折的MRI影像学表现及其临床应用。方法对我院在2012年5月至2014年3月收治的46例创伤性细微骨折患者的MRI影像及临床表现进行分析,统计骨折的部位、形态以及MRI信号等。结果本次研究的46例外伤患者通过MRI检查均显示有细微骨折或骨挫伤,其病灶总计67个。病灶在TlWI、T2WI及STIR不同的序列上清晰显示率分别为95.66%、63.04%、100%,T1WI及STIR与T2WI相比(χ2=11.5907、14.6667,P<0.05),STIR与T1WI相比(χ2=1.0154,P>0.05)。且MRI主要表现为线条形、地图样或者不规则的斑片状SE(T1WI)低信号,FSE(T2WI)序列表现为等或者稍高的信号区域,其中心最高;STIR表现为明显的较高信号影,边界比较清楚。结论对于创伤性细微骨折来说,MRI检查具有较高的敏感性,尤其是脂肪抑制序列的应用,有明显的价值;能较清晰得显示病变位置、数目和骨折程度,为诊断和治疗提供准确临床依据,有利于预后,能够有效减少或者避免误诊和漏诊,具有重要的临床应用价值。 Objective To analysis and study of traumatic bone MRI imaging findings and its clinical application. Methods In our hospital from May 2012 to March 2014 period were treated 46 patients with traumatic fracture of subtle MRI imaging and clinical manifestations were analyzed, statistical fracture location, shape and MRI signals. Results 46 cases of traumatic patients in this study by MR/examination showed subtle fractures or bone contusion, its total 67 lesions. Clear focus on the different sequences T1WI, T2WI and STIR display rate of 95.66%, 63.04% and 100%, T1WI and T2WI compared with STIR(χ2=11.5907, 14.6667, P〈0.05), STIR compared with T1WI(χ2=1.0154,P〉0.05). And MRI mainly for the line shape, map-like or irregular patchy SE (T1WI) low signal, FSE (T2WI) or the signal sequence and other regions showed slightly higher, its highest centre; STIR showed significantly higher signal intensity, the boundary more clearly. Conclusion Traumatic small fracture, MRI examination has a high sensitivity, especially the application of fat suppression sequences, there is obvious value, can clearly display the location of lesions, provide clinical basis for the diagnosis and treatment, is beneficial to the prognosis, to avoid misdiagnosis and missed diagnosis or effective reduction, which has important value in clinical application.
