组织多普勒TDI-Tei指数对慢性肺心病患者右室功能的评价

Evaluation of the Right Ventricular Function in Patients with Chronic Pulmonary Heart Disease using TDI-Tei Index

目的 探讨组织多普勒TDI-Tei指数评价慢性肺源性心脏病患者右心室功能的临床应用价值。方法 选取69例慢性肺源性心脏病患者,根据肺动脉压力升高情况分为3组：重度肺动脉高压组（〉80 mm Hg）21例,中度肺动脉高压组（60-80 mm Hg）33例和轻度肺动脉高压组（〈60 mm Hg）15例;同期在我院进行健康体检的70例健康人作为对照组。对两组研究对象的常规超声指标和右室TDITei指数进行对比研究。结果 与正常对照组相比,常规超声指标只在重度肺动脉高压组中出现明显变化,具有统计学意义（P〈0.05,P〈0.01）,而右室TDI-Tei指数在轻、中、重度三组不同的肺动脉高压组中均大于正常对照组,具有统计学意义（P〈0.05,P〈0.01）。结论 组织多普勒TDI-Tei指数可较传统超声指标更敏感的反映慢性肺源性心脏病患者的右心室功能,具有很好的临床应用价值。

Objective To investigate the Clinical value of Tei index derived from Tissue Doppler imaging（TDI） in assessing the fight ventricular function in patients with chronic pulmonary heart disease. Methods 60 patients were divided into 3 sub-groups： mild（〈60 mm Hg,15 cases）, moderate（60-80 mm Hg, 33cases） and severe（〉80 mm Hg, 21 cases） pulmonary hypertension group according to the PASP. All subjects were measured with the traditional ultrasonic parameters and Tei index by TDI. Results Compared to normal group ,the traditional ultrasonic parameters had changed significantly only in severe pulmonary hypertension group, but RV Tel index of mild,moderate and severe pulmonary hypertension group increased significantly（P〈0.05, P〈0.01）. Conclusion Tel index can be more sensitive to reflect the changes of fight ventricular function in patients with chronic pulmonary heart disease, it can be used for evaluation of right ventricular function.
