肝癌组织中基质金属蛋白酶表达与侵袭转移的关系研究 被引量:6

摘要 目的检测肝癌患者血液及组织中MMP-2、MMP-9的表达,分析二者与肝癌侵袭转移的关系。方法选取2013年1月至2014年10月在我院住院行手术切除的肝细胞癌(HCC)患者40例作为试验组,经病理确诊为HCC,且未经放、化疗等抗肿瘤治疗;对照组患者40例,包括局灶性增生性结节16例,肝血管瘤13例,肝外伤11例。采用酶联免疫吸附法和免疫组织化学法检测入组患者血液及组织中基质金属蛋白酶(MMP)-2、MMP-9的表达。结果采用酶联免疫吸附法对两组患者MMP-2、MMP-9的水平进行检测,结果发现,试验组患者血液中MMP-2水平(0.384±0.026)、MMP-9水平(0.337±0.012)明显高于对照组,差异有统计学意义(P<0.05);采用免疫组织化学法对试验组肝癌组织、癌旁组织、对照组进行MMP-2、MMP-9的检测,结果发现,肝癌组织中MMP-2强阳性(+++)表达30例,MMP-9强阳性(+++)表达31例,阳性率为87.5%、85.0%;癌旁组织无MMP-2、MMP-9强阳性表达,阳性率分别为10.0%、12.5%;对照MMP-2、MMP-9阳性率分别为7.5%、10.0%。试验组肝癌组织MMP-2、MMP-9的表达阳性率较癌旁组织及对照组明显升高,差异有统计学意义(P<0.05);MMP-2、MMP-9的表达与静脉浸润、肝内外转移、包膜有关(P<0.05),与性别、年龄、病灶大小、肝硬化、甲胎蛋白(AFP)、HBs Ag无关(P>0.05)。结论 MMP-2、MMP-9在肝癌中表达升高,与有无静脉浸润、有无肝内外转移及有无包膜有关,并且出现静脉浸润、有肝内外转移、无包膜的肝癌中,MMP-2、MMP-9表达明显升高,与肝癌的生长、侵袭和转移有密切的关系。 ObjectiveTo detect liver cancer patient’s blood and tissue in the expression of MMP-2 and MMP-9, analyze the relationship with the invasion and metastasis of hepatocellular carcinoma. MethodsChose between January 2013 and October 2013 hospitalized in our hospital surgical excision of 40 patients with HCC as experimental group, the pathological diagnosis was HCC, and without radiotherapy, chemotherapy, such as anti-tumor treatment. 40 patients were the control group, including focal proliferative nodules in 16 cases, 13 cases of liver hemangioma, 11 cases of liver trauma. Using enzyme-linked immunosorbent method to test the patient‘s blood and immune histochemical method to test the expression of MMP-2 and MMP-9.ResultsUsing enzyme-linked immunosorbent method on two groups of patients for the level of MMP-2 and MMP-9, the results showed that the blood level of MMP-2 was 0.384±0.026, MMP-9 levels was 0.337±0.012 in the experimental group patients, significantly higher than the control group, the difference was statistically significant (P〈0.05);By immune histochemical method on experimental liver cancer tissue, the tissue adjacent to carcinoma, the control ofMMP-2 and MMP-9, the results showed in the cancer of the liver tissue, MMP-2 strong positive expression (+++) were 30 cases, MMP-9 strong positive expression (+++) were 31 cases, positive rate was 87.5%, 85.0%; tissue adjacent to carcinoma were without MMP-2 and MMP-9 strong positive expression, the positive rate was 10.0%, 12.5%, respectively; MMP-2 and MMP-9 positive rate in control group was 7.5%, 10.0%, respectively. The expression of MMP-2 and MMP-9 positive rate in experimental liver cancer tissue was obviously higher than tissue adjacent to carcinoma and the control group, the difference was statistically significant (P〈0.05); the expression of MMP-2 and MMP-9 related to venous invasion, transfer of liver inside and outside, pericardium (P〈0.05), and were irrelevant to the gender, age, focal size, liver cirrhosis, AFP, HBsAg (P〉0.05).ConclusionMMP-2 and MMP-9 are high expression in liver cancer, and relate to presence of vein invasion, presence of transfer, and the presence of envelope,and with vein invasion, intrahepatic metastasis, without capsule of liver cancer, MMP-2 and MMP-9 expression obviously rises, is irrelevant to growth, invasion and metastasis of liver cancer.
