中药健脾益气汤治疗糖尿病胃轻瘫78例疗效观察

Jianpi Yiqi Decoction in Treating Diabetic Gastroparesis Clinical Observation of 78 Cases

目的观察中药健脾益气汤治疗糖尿病胃轻瘫的临床疗效。方法 2013年6月—2014年12月,该科门诊及住院糖尿病胃轻瘫患者共155例,随机将患者分为观察组78例和对照组77例,两组均严格糖尿病饮食,采取口服降糖药或(和)胰岛素控制血糖在理想水平。观察组采用中药健脾益气汤:方剂中黄芪50 g,党参15 g,山药15 g,白术15 g,茯苓15 g,枳壳10g,半夏10 g,木香9g,鸡内金10g,石斛10 g,丹参20 g,黄芩10 g。1剂/d,每次取汁200 m L,分2次饭后服。对照组:口服多潘立酮,10 mg/次,3次/d,餐前30 min服用。两组疗程均为4个周,4周后观察消化道症状改善程度。结果治疗4周后两组临床疗效比较,对照组77例中,临床治愈27例(35.1%),临床好转31例(40.3%),临床未愈19例(24.7%),临床总有效率为75.3%(58/77),观察组78例中,临床治愈65例(83.3%),临床好转9例(11.5%),临床未愈4例(5.1%),临床总有效率为94.9%,明显优于对照组(P<0.05)。对照组空腹血糖水平为(5.8±1.7)mmol/L,观察组为(5.4±1.3)mmol/L,较对照组低(P<0.05)。结论健脾益气汤有健脾益气、养阴和胃、消胀除满的功效,可有效缓解糖尿病胃轻瘫的消化道解症状。

Objective To observe the effect of Jianpi Yiqi Decoction to treat the diabetes gastroparesis clinical curative effect.Methods From June 2013 to December 2014, the data of 155 patients in our department of outpatient and inpatient with diabetic gastroparesis, were randomly divided into observation and control group.the observation group has 78 cases and the control group has 77 cases, two groups were strictly a diabetic diet, oral hypoglycemic agents or take（and） insulin to control blood sugar in the ideal level.The observation group used traditional Chinese medicine decoction prescription： Astragalus 50 g, Dangshen 15 g, Yam 15 g, Atractylodes 15 g, Poria 15 g,Citrus aurantium 10 g,Pinellia 10 g, Woody 9g, Gallus gallus domesticus 10 g, Dendrobium 10 g, Salvia 20 g, Scutellaria 10 g. 1 agent a day, take juice 200 ml each time, 2 times after meals. Control group： oral domperidone, 10 mg each time, 3 times a day, 30 minutes before meals. 2 groups were treated for 4 weeks, 4 weeks after the observation of gastrointestinal symptoms improved degree. Results To compare the clinical efficacy of two groups after 4 weeks of treatment, 77 cases in the control group, clinical cure in 27 cases（35.1%）, clinical improvement in 31 cases（40.3%）, 19 cases of clinical healed（24.7%）, the total clinical effective rate was 75.3%（58/77）, the observation group of 78 cases, 65 cases were clinically cured（83.3%）, clinical improvement in 9 cases（11.5%）, 4 cases of clinical healed（5.1%）, the total clinical effective rate was 94.9%, significantly better than the control group, P〈0.05. Control group, fasting blood glucose level was（5.8±1.7） mmol/L, the observation group（5.4 ± 1.3） mmol/L, lower than that in control group, P〈0.05. Conclusion strengthening the spleen and replenishing Qi Decoction of strengthening spleen and replenishing qi, nourishing yin and stomach, Xiaozhang in full effect, can effectively alleviate the diabetic gastroparesis symptoms of digestive solution.
