血尿酸与2型糖尿病及糖尿病肾病关系的研究

The Research on the Relationship Between Uric Acid and Type 2 Diabetic Nephropathy

目的研究2型糖尿病患者的血尿酸、糖尿病肾病之间的关系。方法选取该院自2012年1月—2014年1月间收治的300例2型糖尿病患者,将其分为血尿酸组与正常尿酸组,150例/组,检查两组的血压、糖化血红蛋白、人体测量学指标、血脂、血尿酸、尿蛋白排泄率以及血肌酐等指标,计算肾小球的滤过率,分析肾小球滤过率和尿蛋白排泄水平。结果尿酸组中无蛋白尿患者,大量白蛋白尿患者以及微量白蛋白尿患者的肾小球滤过率均较之正常组低且差异有统计学意义(P<0.05)。尿酸组中大量蛋白尿的患者患病率明显高于正常组,差异有统计学意义(P<0.05);而两组间微量蛋白尿患病率,差异无统计学意义(P>0.05)。结论 2型糖尿病患者血尿酸与糖尿病肾病相关,血尿酸能够直接反映出肾小球滤过率下降水平,值得临床诊断推广和应用。

Objective To study the relationship between blood uric acid in patients with type 2 diabetes mellitus, diabetic nephropathy. Methods selected in our hospital from 2012 January-2014 year in January admitted 300 cases of type 2 diabetes patients, divided into the blood uric acid group and normal uric acid group, 150 patients per group, two groups were examined blood pressure, hemoglobin A1 c, anthropometry, blood lipid, serum uric acid, urinary protein excretion rate and blood creatinine glomerular filtration rate, calculation, analysis of glomerular filtration rate and urinary protein excretion. Results no proteinuria of patients with uric acid group, patients with macroalbuminuria and microalbuminuria in patients with glomerular filtration rate were compared with the normal group is low and the difference was significant（P〈0.05）, with statistical significance. A large number of protein in the urine uric acid group of patients the prevalence was significantly higher than normal group（P〈0.05）,with statistical significance between the two groups; and the prevalence of microalbuminuria did not show significant difference（P〉0.05）,no statistical significance.ConclusionPatients with type 2 diabetes and diabetic nephropathy associated with uric acid, uric acid can directly reflect the level of decline in glomerular filtration rate, worthy of promotion and application of clinical diagnosis.
