临床路径管理在糖尿病患者中的应用

Application of Clinical Pathway Management in Patients with Diabetes

目的分析糖尿病患者实施临床路径管理的效果。方法选取该院2013年200例糖尿病患者,患者不实施临床路径管理;另外在2014年选取200例糖尿病患者,患者实施临床护理管理。对临床路径管理干预前后患者临床指标改善效果对比。结果经过干预患者血糖均值和收缩压和干预前相比,有显著下降,差异有统计学意义(P<0.05);糖化血红蛋白偏高患者干预后空腹血糖及糖化血红蛋白显著低于干预前,差异有统计学意义(P<0.05);糖化血红蛋白正常患者,干预后糖化血红蛋白和空腹血糖和干预前相比显著偏高,差异有统计学意义(P<0.05)。结论临床路径管理有助于强化糖尿病患者血糖控制,提高患者生活质量,值得推广。

Objective To analyze the effect of the implementation of clinical pathway management in patients with diabetes mellitus.Methods in our hospital in 2013 200 cases of diabetic patients,not the implementation of clinical pathway management in selected patients;200 patients with diabetes in 2014,with the implementation of clinical nursing management.Before and after the intervention of clinical indexes in patients with clinical pathway management improvement effect comparison.Results after intervention in patients with systolic blood pressure and mean blood glucose and intervention compared to before,was significantly decreased（P〈0.05）;the difference was significant（P〈0.05）;glycosylated hemoglobin is higher after the intervention in patients with fasting blood glucose and glycosylated hemoglobin were significantly lower than those before intervention,the difference was significant（P〈0.05）;after the intervention in patients with normal glycosylated hemoglobin,glycosylated hemoglobin and the fasting blood glucose and significantly higher than those before the intervention,the difference was significant（P〈0.05）.Conclusion the clinical pathway management helps to strengthen the control of blood glucose in diabetic patients,improve the quality of life of patients,is worthy of promotion.
