全科团队服务和网络服务管理模式对糖尿病患者生存质量的影响分析

Analysis of General Practitioner Team Services and Management Mode of Network Service Impacting on Quality of Survival of Patients with Diabetes Mellitus

目的:分析联合全科团队服务和网络服务管理模式两种管理模式加强社区糖尿病患者管理后,对糖尿病患者生存质量影响。方法:采取两阶段随机抽样的方法,抽取2013年12月-2014年12月在沿湖、洞庭、七里山、奇家岭和郭镇社区登记的2型糖尿病患者210例,联合2种模式对社区糖尿病患者进行管理,对比分析两组患者的生存质量。结果:管理后,患者空腹血糖下降程度、糖尿病患者的知信行量表总分和各维度的得分增加,DSQL量表总分下降,简明健康状况问卷(SF-36)评分总分较干预前升高,差异有统计学意义。结论:联合两种模式加强对社区糖尿病患者的管理能够进一步提升糖尿病患者生存的质量,值得推广应用。

To analyze two kinds of management patterns of combining general practitioner team service and network service management, after strengthening management of community diabetic patients, impacting on quality of survival of patients with Diabetes Mellitus. Extracting 210 patients with 2-diabetes mellitus registered in Yanhu, Dongting, Qilishan, Qijialing and Guozhen community from December of 2013 to December of 2014 by adopting two stage random sampling method, to manage community diabetes mellitus by combing two modes, which analyze the quality of survival of patients in two groups. After management, the scores that the level of patients with fasting plasma glucose decreasing, score of KAP scale for diabetic patients and each dimension increase; the scores of DSQL scale decrease; the scores of concise health survey questionnaire （SF-36） scoring intervened in advance increase. The differences between them are statistically significant. It is worth using widely that combining two modes strengthens the management of community diabetic patients further improved the quality of the survival for patients with diabetes.
