院外心脏电除颤不同时机选择对心肺复苏的影响 被引量:7

摘要 目的探讨抢救院外心搏呼吸骤停患者应用自动体外除颤器最佳时机,旨在提高除颤和心肺复苏的效果。方法对江苏省南通市急救中心2011年1月~2013年12月救治的60例院外突发心搏骤停进行除颤患者,每次除颤前除常规记录波形外,还记录心搏骤停至心肺复苏启动时间、心搏骤停至院外急救员到达时间、心搏骤停至除颤时间、有/无目击者心肺复苏和心功能分级,回顾分析这些因素与除颤成功率的关系,进一步得到除颤最佳时机。结果不同心搏骤停至心肺复苏启动时间、心搏骤停至院外急救员到达时间、心搏骤停至除颤时间的除颤成功数和自主循环恢复数不同,时间越短除颤成功数和自主循环恢复数越高;有旁观者复苏24例,除颤成功17例占70.8%;无旁观者复苏36例,除颤成功仅6例;波振幅或频率越高除颤成功率越高;心功能美国纽约心脏病学会分级逐渐增加,除颤成功率逐渐下降,组间比较差异均有统计学意义。结论心肺复苏启动时间越早,自动体外除颤器获得时间越短,除颤时机越佳;室颤波振幅和频率较高,除颤时机较好;心功能分级较差,除颤时机较差。
