我院药品风险管理实践

Practice of Drug Risk Management in Our Hospital

目的:实施药品风险管理,保障医院临床用药安全。方法:通过药品风险识别、风险评估、风险控制和风险审核建立我院药品风险管理模式,制订并实施风险管理措施;通过收集和分析3年内(2012-2014年)向临床医护人员和患者发放的调查问卷结果及院内药事管理和药物使用质控小组的考核,评价药品风险管理的实施效果。结果:通过建立并完善药品管理信息系统、强化特殊药品的管理、严格执行《退货、药品召回的管理制度》、设立专门的药品安全监测机构、院内自查等措施,我院2012-2014年与药品安全相关的问题逐年减少(分别为88、42、19件);院内药事管理和药物使用质控小组对药房处理药品安全问题工作的考核合格率分别为96.59%、97.62%、100%。结论:我院建立的药品风险管理模式促进了院内药品的安全使用。

OBJECTIVE：To carry out drug risk management so as to ensurethe safety of clinical drug use in the hospital.METHODS：The mode of drug risk management of our hospital was established,in the form of drug risk identification,risk evaluation,risk control and risk review,and the measures for risk management were developed and taken.The effect of drug risk management was evaluated after 3 years（2012-2014）of colletion and analysis of the questionnaires given to clinical medical workers and patients and the results of evaluation by the group of pharmacy management and quality control for drug use in the hospital.RESULTS：Following the establishment and improvement of drug management information system,the strengthening of the management of special drugs,strict implementation of the Management System for the Return and Recall of Drugs,the establishment of specialized drug safety monitoring body,the development and implementation of the measures such as self-inspection in the hospital,the problems about drug safety in our hospital had been decreasing from 2012 to 2014（88,42 and 19 cases respectively）;and96.59%,97.62% and 100% of drug safety problems were solved by the pharmacy staff to the satisfaction of the group of pharmacy management and quality control for drug use in the hospital respectively in the three years.CONCLUSIONS：The mode of drug risk management established in our hospital has promoted the safety of drug use.
