补肺益肾法对慢性阻塞性肺疾病稳定期患者的疗效及生活质量影响的临床研究

Clinical Study on Curative Effect and Quality of Life in Patients with Chronic Obstructive Pulmonary Disease Treated by Bufei Yishen Method of TCM

目的:探讨补肺益肾法组方配合常规西药对慢性阻塞性肺疾病稳定期患者的疗效和生活质量的影响。方法:将64例符合纳入标准的病例按随机数字表法分为治疗组和对照组,治疗组以补肺益肾法组方配合沙丁胺醇气雾剂,对照组以沙丁胺醇气雾剂治疗,治疗3月为1疗程。在治疗前、后和随访第3月的COPD患者呼吸困难程度、肺功能测定、圣乔治呼吸问题调查问卷和健康调查简表进行评分和记录。采用SPSS 17.0软件包进行统计分析。结果:对呼吸困难评分肺功能及运动能力评价中,补肺益肾法组方配合西药进行治疗,可以使呼吸困难评分、肺功能参数FEV1、FEV1/FVC得到较好改善(P<0.05);且治疗后,治疗组中的肺功能参数FEV1要明显优于对照组(P<0.05)。对SGRQ生活质量评分的评价中,治疗组对改善呼吸症状、活动受限、疾病影响、总评分具有较好作用(P<0.05);且治疗组中活动受限、疾病影响两项指标优于对照组(P<0.05)。在对SF-36生活质量评分的评价中,治疗组对改善生理功能、生理职能、总分效果比对照组好(P<0.05或P<0.01);随访中,治疗组中的生理功能、生理职能、精神健康、总分要明显优于对照组(P<0.05或P<0.01)。结论:肺益肾法组方配合常规西药可以改善稳定期COPD患者患者的呼吸困难评分、肺功能、SGRQ生活质量、生活质量评分。

Objective：To investigate the Bufei Yishen Recipe combined conventional western medicine efficacy and quality of life in patients with stable chronic obstructive pulmonary disease. Methods ：64 patients who met the inclusion cri- teria were randomly divided into treatment group and control group. The treatment group was treated with Bufei Yishen recipe combined with salbutamol aerosol and the control group with salbutamol aerosol therapy. 3 months was a course of treatment. We scored and recorded pulmonary function in COPD patients and the St Georgeg respiratory problems,ques- tionnaire and short form health survey after following up for 3 months The statistical analysis used SPSS 17 software. Re- sults ：For the evaluation of dyspnea score,lung function and exercise capacity,Bufei Yishen Recipe combined with western medicine treatment can improve the score and the lung function parameters such as FEV1 and FEV1/FVC （P 〈 0.05 ）. After treatment in treatment group, FEV1 was significantly better than that of control group （P 〈 0.05 ）. As for the evalu- ation of the quality of life by SGRQ, the treatment group had better effects on improving respiratory symptoms, activity lim- itation,disease influence and the total score （P 〈 0.05 ） and the limited activity and the disease affecting in treatment group were better than those of the control group （P 〈 0.05）. In the evaluation on the quality of life by SF36 score, the treatment group can improve the physiological function and the total effect was better than the control groupg （P 〈 0.05 or P 〈0.01 ）. During the follow- up period, the treatment group＇s physiological function, physiological function, mental health and the total score were significantly better than those of control group （ P 〈 0.05 or P 〈 0.01 ）. Conclusion： The Bufei Yishen method of TCM combined with conventional western medicine can improve the patients with stable COPD in the dyspnea score,lung function, SGRQ of quality of life and the score of quality of life.
