摘要
目的研究MRI对诊断中枢神经系统白血病的诊断价值。方法回顾性分析2011年3月-2014年5月243例经临床确诊为白血病,并先后分别行头颅、全脊柱MRI增强扫描及脑脊液细胞学检查的病例,分为出现中枢神经系统症状或体征组、无中枢神经症状组。结果 CNSL的MRI检查可表现软脑(脊)膜浸润、硬脑(脊)膜浸润、脑(脊髓)实质侵犯及上述类型混合型。243例病例中,MRI有阳性发现为27例;CSF细胞学检查阳性20例。27例MRI有阳性发现的病例中,CSF细胞学检查阳性15例。另有5例为CSF细胞学检查阳性,但MRI无阳性发现。结论 MRI对发现CNSL有良好的敏感性,并可有效地检出CSF细胞学检查为阴性的CNSL病例,与CSF细胞学联合运用可降低临床CNSL的假阴性率。
Objective To investigate the value of MRI in the diagnosis of central nervous system(CNS) leukemia.Methods Head and spinal MRI findings and cytology of cerebrospinal fluid(CSF) of 243 patients with leukemia were analyzed retrospectively. Results The MRI features of CNS leukemia include infiltration of brain or spinal pia mater or dura mater, invasion of brain or spinal cord. There were MRI was positive in 27 / 243 and CSF cytology was positive in 20 / 243 patients including 15 / 27 with MRI findings whereas MRI was normal on the remaining 5 patients with positive CSF cytology. Conclusion MRI is sensitive for diagnosing CNS leukemia and can reduce the false negative rate when combined with CSF cytology.
