摘要
目的探讨子宫输卵管造影(HSG)中,双腔球囊导管顶端位置的不同对该侧输卵管显示率的影响。方法选择HSG检查中导管顶端紧邻子宫角部且无明显间隙,当对比剂注入4~5 ml或对侧输卵管全程对比剂显影而该侧输卵管仍未见对比剂显示的受检者72例,按不同操作方法随机分为A、B、C三组进行对比研究。A组(23例):保持原状态,即不调整双腔球囊管的位置及球囊大小继续注药;B组(30例):放出球囊气体约0.5~1 ml后继续注药,以导管顶端周缘出现对比剂为度;C组(19例):在继续注药的同时轻拉导管,以导管不脱出为度。每次拉动持续时间不少于1 s,每拉动3次为1个观察周期。摄片观察、记录双侧输卵管的显示情况并作组间对照。数据分析采用SPSS 12.0进行卡方检验。结果 A组显示通畅率为30.43%(7/23),B组为76.67%(23/30);C组为63.16%(12/19)。A、B两组间对照,差异均有显著统计学意义(χ^2=11.328,P=0.001);A、C两组间对照,差异均有统计学意义(χ^2=4.497,P=0.034);B、C两组间对照,差异无统计学意义(χ^2=1.040,P=0.308)。结论当导管顶端紧邻子宫角部时,适当减小球囊体积或向外轻拉导管是提高输卵管显示率的有效方法。
Objective To analysis effects of the position of the double-lumen catheter's top end in displaying the same side fallopian tube in hysterosalpingography(HSG).Methods The 72 patients according with the standard that there is no interspace between the head of catheter and one side uterine horn were selected in our experiment.All cases were randomly divided into 3 groups.Group A(23cases) to be keeped injecting media without any adjustment; Group B(30cases) to be inject media after 0.5-1 ml air of the balloon out; Group C(19cases) bo be inject media as well as gently pulling out the double-lumen catheter at the same time.Observe and record the situation of display of fallopian tube and do comparison in the groups.The data was analyzed by chi-square(SPSS12.0).Results Comparison and analysis between the group A and group B.The difference between groups were statistically significant(χ^2= 11.328,P = 0.001); Comparison and analysis between the group A and group C,the difference between groups were statistically significant(χ^2= 4.497,P = 0.034).Comparison and analysis between the group B and group C,the difference were not statistical significant between the groups(χ^2= 1.040,P = 0.308).Conclusion When the double-lumen catheter's top end close to one side Uterine horn,properly reducing the volume of the balloon volume and pulling out the catheter can increase the display rate of one side fallopian tube in HSG.
