肾脏小肿瘤的MRI诊断与鉴别诊断

MRI diagnosis and differential diagnosis of kidney small tumors

目的分析直径≤4cm的肾透明细胞癌34例、乏脂肪肾血管平滑肌脂肪瘤18例的MRI表现,以提高肾脏肿瘤的MRI诊断及鉴别诊断水平。方法回顾性分析经手术病理及长期随访证实的52例直径≤4cm的肾脏肿瘤性病变的MRI资料。观察并记录病灶形态、边界、各序列信号、IP值/OP值及ADC值,计算rADC值及STII值,并进行统计学分析。结果病灶中心位置、与肾皮质交角、T2WI信号强度等研究指标在两组病例中差异均具有统计学意义,而两组病灶的强化方式各异,且多有重合。结论 MRI对直径≤4cm的肾脏小肿瘤的鉴别诊断具有重要意义,仔细分析其信号特征有助于做出准确的术前诊断。

Objective To analyse the MR performance of 34 renal clear cell carcinoma and 18 renal angiomyolipoma with minimal fat （in diameter of 44 cm） ao as to improve the MR diagnosis and differential diagnosis of kidney tumors. Meth- ods The MR performance of fifty-two renal tumors （in diameter of ≤4 cm） confirmed by pathology or longer-term fol- low-up were retrospectively evaluated, including thelesion shape, boundary, signal, IP/OP value, ADC values, rADC val- ue and STII value. Results The features had significant differences between renal clear cell carcinoma and kidney angio- myolipomas （ P 〈0.05）, including location of lesion center, angle of intersection, T2WI, etc. The MRI dynamic contrast enhanced of the two groups were varied and had a lot overlaps. ConcLusion MRI may be useful in differentiating renal small tumors.
