全胸腔镜肺叶切除术治疗周围型肺癌50例临床分析

Clinical Analysis of 50 Cases Under the Treatment of Peripheral Lung Cancer with Video-assisted Thoracic Surgery

目的探讨胸腔镜肺叶切除术(VATS)治疗周围型肺癌的临床效果。方法选择周围型肺癌患者100例,其中50例行VATS治疗,列为VATS组,另50例行开胸手术(TOS)治疗,列为TOS组,比较两组患者围手术期资料、术后并发症及1年、3年复发率、生存率。结果 VATS组患者较TOS组手术时间、术后引流时间、卧床时间及住院时间短,失血量少,术后VAS评分低,两组比较差异有统计学意义(P<0.05);两组患者术后并发症、1年和3年复发率及生存率比较差异无统计学意义(P>0.05)。结论胸腔镜肺叶切除术治疗周围型肺癌具有较高安全性,且具有创伤小、恢复快的临床优势。

Objective To discuss the effect of the treatment of peripheral lung cancer with video- assisted thoracic surgery（ VATS）.Methods Selecting 100 patients with the peripheral lung cancer,of which 50 cases were under VATS treatment as the VATS group,and another 50 cases were under thoracic surgery（ TOS） treatment as TOS group. Perioperative data,complications and 1- year and 3- year recurrence rate of survival of the patients of the two groups were compared. Results Patients in VATS group had less less postoperative drainage time,bed time and shorter hospital stay,blood loss and postoperative VAS score than TOS group,with statistically significant difference（ P〈0. 05）; There were significant statistical meaning of ostoperative complications,1- year and 3- year recurrence rate and survival rate（ P〉0. 05）. Conclusion Video- assisted thoracic surgery of the treatment of peripheral lung cancer has higher security,with clinical advantage of less trauma and quicker recovery.
