摘要
目的探讨NAH方案治疗对高血压合并代谢异常患者的血液流变学的影响。方法收集500例原发性高血压患者,依据是否合并代谢异常将患者分为单纯高血压组、高血压合并血脂紊乱组、高血压合并糖尿病组、高血压合并腹型肥胖组及高血压合并代谢综合征组,每组患者各100例。对所有患者进行NAH方案治疗,比较治疗10个月后各组患者降压达标率(血压<140/90 mm Hg)、血液流变学指标及不良反应。结果采用NAH方案进行治疗后,单纯高血压组、高血压合并腹型肥胖组的降压达标率较高,血流动力学指标(全血粘度低切、全血粘度高切、血浆黏度及红细胞聚集指数)大致恢复正常范围,不良反应发生率低,与高血压合并血脂紊乱组、高血压合并糖尿病组及高血压合并代谢综合征组比较差异具有统计学意义(P<0.05)。结论 NAH方案治疗对单纯高血压及高血压合并腹型肥胖患者具有较好的治疗效果,不良反应较少,血流动力学变化小。
Objective To evaluate the effect of NAH on haemodynamics in hypertension patients with metabolic abnormalities.Methods Selecting 500 patients with primary hypertension,according to whether combined with metabolic abnormalities,dividing the patients into simple hypertension group,hypertension combined with dyslipidemia group,hypertension combined with diabetes group,hypertension combined with abdominal obesity group,hypertension combined with metabolic syndrome group,with100 patients in each group.All patients were treated with NAH solution. After 10 months treatment,the step- down success rate( blood pressure 140 /90 mm Hg),haemodynamics and adverse reaction were compared among each group. Results After NAH solution treatment,the step- down success rate of simple hypertension group and hypertension combined with abdominal obesity group significantly increased,haemodynamics( whole blood low shear viscosity,whole blood high shear viscosity,plasma viscosity and erythrocyte aggregation index) roughly recovered the normal range,adverse reaction was less,and the differences of hypertension combined with dyslipidemia group,hypertension combined with diabetes group and hypertension combined with metabolic syndrome group showed statistically significant( P〈0. 05). Conclusion Using the NAH solution treats with patients for simple hypertension and hypertension combined with abdominal obesity has good clinical effect,with less adverse reaction and slight hemodynamic changes.
