急性ST段抬高型心肌梗死患者白细胞与肌钙蛋白I和N端B型脑钠肽前体的关系

摘要 目的探讨急性ST段抬高型心肌梗死(STEMI)患者外周血白细胞(WBC)计数与肌钙蛋白I(c Tn I)、N端B型脑钠肽前体(NT-pro BNP)的关系。方法选择STEMI患者146例,依据白细胞计数水平分为白细胞升高组(WBC>10×109/L)和白细胞正常组(WBC≤10×109/L),比较两组患者的临床特征,测定两组患者c Tn I、NT-pro BNP水平并进行比较。结果与WBC正常组比较,WBC升高组患者的c Tn I、NT-pro BNP水平显著升高,WBC升高程度与c Tn I、NT-pro BNP水平呈正相关。结论 STEMI患者的外周血白细胞水平与c Tn I、N端B型脑钠肽前体水平密切相关,WBC作为炎症指标可间接反映心肌梗死的面积和程度。 Objective To evaluate the correlation between peripheral white blood cell( WBC) count and cardiac troponin I( c Tn I) and N- terminal pro- B- type natriuretic peptide( NT- pro BNP) in acute ST- elevated myocardial infarction( STEMI) patients. Methods A total of 146 patients with acute STEMI were categorized into WBC elevated group( WBC 10 × 109/ L) and WBC normal group( WBC≤10× 109/ L) according to the levels of WBC. Clinical characteristics were compared between the two groups. The levels of WBC,c Tn I and NT- pro BNP were measured in all patients. Results Compared with WBC normal group,c Tn I and NT- pro BNP levels were significantly elevated in elevated group. There were the positive correlation between WBC count and c Tn I,and NT- pro BNP levels. Conclusion The WBC level of acute STEMI patients is closely associated with c Tn I and NT- pro BNP levels. WBC as inflammatory markers may indirectly reflect the size and severity of myocardial infarction with acute STEMI patients.
