高频振荡通气治疗新生儿呼吸窘迫综合征效果评价

摘要 目的探讨高频振荡通气(HFOV)对新生儿呼吸窘迫综合征(NRDS)的治疗效果。方法选取2012-04~2014-05间我院收治的80例NRDS患儿,将其随机分为观察组和对照组。对照组采用常频机械通气(CMV)治疗,观察组采用HFOV治疗,比较两组患儿的治疗效果及并发症。结果观察组机械通气24 h后呼吸机参数Fi O2、MAP、OI值均低于对照组,血气分析中PCO2值高于对照组,两组比较差异有统计学意义(P〈0.05);观察组吸氧时间、机械通气时间及住院时间均短于对照组,差异有统计学意义(P〈0.05);观察组并发症发生率为2.50%,明显低于对照组(20.00%),差异有统计学意义(P〈0.05)。结论高频振荡通气治疗新生儿呼吸窘迫综合征,可改善患儿通气,缩短机械通气时间,减少并发症发生,具有显著临床治疗效果。 Objective To investigate the treatment effect of high frequency oscillation ventilation( HFOV) for neonatal respiratory distress syndrome( NRDS). Methods 80 neonates with NRDS were selected from the hospital from April,2012 to May,2014,and were assigned to the observation group and the control group randomly. The observation group were treated with HFOV while the control group were treated with conventional mechanical ventilation( CMV). The treatment effect and complication were compared between the two groups. Results The value of Fi O2,MAP and OI of observation group after 24 hours mechanical ventilation were all lower than those of control group,while the value of PCO2 of observation group was higher than that of control group,with statistical significance( P〈0. 05).The duration of oxygen uptake,mechanical ventilation and hospitalization of observation group were shorter than those of control group( P〈0. 05). The complication incidence of observation group and control group were 2. 50% and 20. 00%,respectively. The complication incidence of observation group was apparently lower than that of control group( P〈0. 05). Conclusion HFOV in treatment of NRDS can improve the ventilation of neonates,shorten the duration of mechanical ventilation and reduce the complication incidence,which has a significant clinical treatment effect.
