摘要
目的探析N末端脑钠肽原(NT-ProBNP)对基层不稳定型心绞痛患者危险分层的价值研究。方法选取我院2013年6月-2014年6月符合条件的不稳定型心绞痛患者186例,对入院患者均在出入院时检测NT-proBNP,之后随诊1年。观察入院时NT-proBNP正常组(A组)和升高组(B组)的心血管不良事件(MACE)的发生(主要指标包括心绞痛反复发作、心肌梗死、恶性心律失常、失代偿心衰、猝死等)情况;再观察出院时NT-proBNP正常组(C组)和升高组(D组)的心血管不良事件(MACE)的发生率。结果 A组的心绞痛反复发作、心肌梗死、恶性心律失常、失代偿心衰、猝死等心血管事件的发生情况显著低于B组,差异有统计学意义(P<0.05);C组的心绞痛反复发作、心肌梗死、恶性心律失常、失代偿心衰、猝死等心血管事件的发生情况显著低于D组,差异有统计学意义(P<0.05)。结论 NT-proBNP可以作为不稳定型心绞痛患者预后的独立预测因子,指导基层临床医生对不稳定型心绞痛患者进行危险分层,并采取不同的治疗策略,NT-pro BNP对基层不稳定型心绞痛患者的危险分层有重要价值。
【Objective】To study the value of NT-Pro BNP for grass-root clinicians to make risk stratification in patients with unstable angina. 【Methods】186 patients with unstable angina treated from June 2013 to June 2014 in our hospital were selected, received the detection of NT-Pro BNP and adopted one-year periodic checkups. For normal group(Group A) and increased group(Group B), the incidences of MACE(including recurrent attacks of stenocardia, MI, malignant arrhythmia, decompensation of heart failure, sudden death) were observed when the patients were admitted to hospital; for normal group(Group C) and increased group(Group D), the incidences of MACE were observed when the patients were discharged from hospital. 【Results】The incidences of MACE including recurrent attacks of stenocardia, MI, malignant arrhythmia, decompensation of heart failure, sudden death of Group A were significantly lower than those of Group B(P〈0.05); the incidences of MACE including recurrent attacks of stenocardia, MI, malignant arrhythmia, decompensation of heart failure, sudden death of Group C were significantly lower than those of Group D(P〈0.05). 【Conclusion】NTpro BNP, as the independent predictive factors of prognosis in patients with unstable angina can guide the grass-root clinicians to make risk stratification and adopt the different treatment strategies. NT-pro BNP has an important value for grass-root clinicians to make the risk stratification in patients with unstable angina.
