脑卒中后倾斜综合征动平衡特征探讨 被引量:3

摘要 目的为脑卒中后倾斜综合征的治疗方法提供生物力学中的动平衡参数依据,从而更有效地矫正倾斜综合征.方法脑卒中后并发倾斜综合征的左侧肢体偏瘫患者和不伴有倾斜综合征的2组患者均通过动态平衡系统中的站立稳定性测试和稳定极限测试模式,分别测量出静态和动态状态下的重心分布及中心控制能力情况,总结出患者的生物力学特征.结果站立位静态稳定性测试显示中心偏离度实验组与对照组相比在A区域的偏离度存在差异性(P<0.05);站立位静态稳定性测试各象限倾斜持续测试中实验组与对照组相比在I、IV象限持续时间存在差异性(P<0.01);站立位稳定极限控制重心的能力测试中实验组与对照组比较在左侧的控制能力具有显著性差异(P<0.01).结论中风后伴有倾斜综合征的患者在站立位静态稳定性测试显示中心偏离度在A区域的控制时间较少,明显偏离中心,在各象限的控制时间上主要集中在I、IV象限,倾斜综合征患者重心主要在左侧;倾斜综合征患者站立位稳定极限测试显示对左侧的重心控制能力较好. Objective To provide the biomechanics evidence for the physical therapy of pusher syndrome in stroke survivors and improve the effectiveness of physical therapy of pusher syndrome. Methods The standing stability model and limits of stability model of the BIODEX balance system were used. The center of gravity (COG) distribution and center control ability were tested both in static and dynamic state in patients with (experiment group) and without pusher syndrome (control group) Results The difference (P〈0.05) of the center control ability of A area was significant when compared two groups in static standing position. The difference of duration (P〈0.01) was showed in I and IV quadrants in the two groups in static standing position. The significant difference was found in left lower limb in the limits of stability test model. Conclusion The duration of control in A area was decreased in the stroke patients with pusher syndrome when compared with patients without pusher syndrome. The duration of control in Ⅰ and Ⅳ quadrants was longer in experiment group. The control ability of COG in experiment group was better in left lower limb according to the result of the limits of stability test model.
