恩替卡韦联合扶正化淤胶囊治疗失代偿期乙肝肝硬化的临床观察

Clinical observation on the treatment of decompensated hepatitis B cirrhosis with entecavir in combination with Fuzheng Huayu capsule

目的观察恩替卡韦联合扶正化淤胶囊治疗失代偿期乙肝肝硬化的临床疗效。方法 60例乙肝肝硬化失代偿期患者随机分为观察组及对照组,每组30例,两组均给予综合护肝及恩替卡韦抗病毒治疗,在此基础上,观察组联合扶正化瘀胶囊抗肝纤维化治疗。观察两组患者的临床症状及肝功能恢复情况、HBV-DNA定量变化、肝纤维化指标变化及并发症情况。结果治疗24、48周后,两组患者的ALT、TBIL水平和HA、LN、PC-Ⅲ水平明显下降,ALB、PAT水平明显升高,Child-Pugh评分明显降低,且以观察组的变化更为明显(P<0.05或0.01);两组治疗后HBV-DNA定量变化及血清HBe Ag转换率比较差异则无统计学意义(P>0.05)。结论恩替卡韦联合扶正化瘀胶囊治疗乙肝肝硬化失代偿期患者较单用恩替卡韦效果更佳,值得临床应用。

Objective To observe clinical efficacy of entecavir in combination with Fuzheng Huayu capsule in the treatment of decompensated hepatitis B cirrhosis. Methods 60 patients with decompensated hepatitis B cirrhosis were randomly divided into Observation Group and Control Group with 30 cases in each group. Both groups were given comprehensive liver protection therapy and antiviral therapy with entecavir. On this basis, Observation Group was given antifibrotic therapy with Fuzheng Huayu capsule. Clinical symptoms and liver function recovery of both groups, HBV-DNA quantitative change, change of indicators of hepatic fibrosis and complications were observed. Results After a treatment for 24, 48 weeks, ALT and TBIL level and HA, LN and PC-Ⅲ level in both groups were significantly decreased, ALB and PAT level in both groups were significantly increased, Child-Pugh score in both groups was significantly decreased, and the change in Observation Group was more significant（P〈0.05 or 0.01）; There was no statistical difference between two groups in terms of HBV-DNA quantitative change and HBe Ag conversion rate after treatment（P〉0.05）. Conclusion The application of entecavir in combination with Fuzheng Huayu capsule has a better efficacy in treating decompensated hepatitis B cirrhosis compared with single application of entecavir, and is worth clinical application.
