Pathologic diagnosis and Gleason grading of the ISUP prostatic carcinoma Gleason grading system by needle biopsy specimens

目的：观察前列腺癌穿刺活检标本病理形态学改变并进行Gleason评分。方法：收集31例前列腺穿刺活检确诊为前列腺癌的标本,采用2005国际泌尿病理协会（ISUP）修订标准进行评分（GS）。结果：Gleason 6分占16.1%（5/31）,7分占61.3%（19/31）,8~10分占22.6%（7/31）。以3级为主要结构占48.4%（15/31）,次要结构占51.6%（16/31）;以4级为主要结构占41.9%（13/31）,次要结构占35.5%（11/31）;以5级为主要结构成分9.7%（3/31）,次要结构成分占12.9%（4/31）。GS=6分占16.1%（5/31）,GS≥7分占83.9%（25/31）。结论：2005国际泌尿病理协会前列腺癌Gleason分级系统,对于穿刺活检标本Gleason评分较高,其中3级和4级为常见结构,3级结构更为常见。修订后的Gleason分级系统判断标准明确,容易掌握,有助于对前列腺癌穿刺标本进行规范分级。

Objective：To observe pathological changes in prostatic carcinoma needle biopsy samples and give Gleason Scores. Methods：31 needle biopsy specimens of diagnosed prostatic cancer patients were collected from Jan. 2010 to Oct. 2014. Gleason scoring and the studies were conducted according to the 2005 refinements of the ISUP. Results： The resuits showed that GS 6,7,8 - 10 were each seen in 16.1% （5/31） ,61.3% （ 19/31 ） ,and 22.6% （7/31） of the cases, respectively . Of which, GS 6 accounted for 16.1% （5.31） and GS≥7 accounted for 83.9% （25/31）. Conclusions： The study showed higher Gleason scores following the 2005 updated ISUP prostate carcinoma Gleason grading system. Pattern 3 and 4 were common patterns, while pattern 3 occurring more often. 2005 ISUP Prostate carcinoma Gleason grading system provided more precise and practical criteria for assigning Gleason scores to needle biopsy specimens, allowing for improved clinical prognosis and treatment.
