二维及彩色多普勒超声对甲状腺癌的诊断价值 被引量:3

The Value of Two-dimensional Ultrasound and Color Doppler Ultrasound in the Diagnosis of Thyroid Cancer

摘要 目的探讨二维及彩色多普勒超声在诊断甲状腺癌中的临床价值。方法选取2009年5月至2013年4月我院收治的50例甲状腺癌患者为观察组,选取同期收治的40例甲状腺腺瘤为对照组,所有患者均经术后病理结果证实。分别采用二维及彩色多普勒超声对患者进行检测,观察并比较二维及彩色多普勒超声对甲状腺癌诊断的符合率,同时比较两组患者二维超声图像、血流丰富程度、动脉收缩期峰值速度(Vmax)及阻力指数(RI)。结果二维超声在甲状腺癌中的诊断符合率为86.00%(43/50),彩色多普勒超声在甲状腺癌中的诊断符合率为92.00%(46/50)。观察组低回声、周边无红晕及微钙化灶的检出率明显高于对照组,差异有统计学意义(P<0.05)。观察组Vmax值及RI值均显著高于对照组,差异有统计学意义(P<0.05);观察组血流丰富程度明显高于对照组(P<0.05),分型为Ⅰ型的患者比例显著低于对照组(P<0.01),分型为Ⅱ型的患者比例显著高于对照组(P<0.01)。结论在二维超声的基础上应用彩色多普勒超声可提高甲状腺癌诊断的准确度,降低误诊率,值得在临床上予以推广。 Objective To explore the clinical value of two-dimensional ultrasound and color Doppler ultrasound in the diagnosis of thyroid carcinoma. Methods 50 patients with thyroid carcinoma admitted to our hospital from May 2009 to April 2013 were selected as observation group, and 40 patients with thyroid adenoma at same period were set as control group, all patients were confirmed by postoperative pathological results. Patients was detected by two-dimensional ultrasound and color Doppler ultrasound respectively. The diagnostic coincidence rate of two-dimensional ultrasound and color Doppler ultrasound in thyroid carcinoma was observed and compared,the two-dimensional ultrasound images, plenty degree of blood flow, artery peak systolic(Vmax) and resistance index(RI) were also compared between two groups. Results The diagnostic coincidence rate of two-dimensional ultrasound in thyroid carcinoma was 86.00%,and the diagnostic coincidence rate of color Doppler ultrasound was 92.00%. The detection rate of low echo, no circumjacent flush and micro-calcification of observation group were significantly higher than those of control group, with statistical difference(P〈0.05). The Vmax value and RI value of observation group were significantly higher than those of control group, with statistical difference(P〈0.05). The plenty degree of blood flow of observation group was significantly higher than that of control group, and the ratio of Ⅰ type patients was lower than that of control group, the ratio of Ⅱtype patients was higher than that of control group(P 0.01). Conclusions The application of color Doppler ultrasound on the basis of two-dimensional ultrasound can improve the diagnostic accuracy of thyroid carcinoma, reduce the misdiagnosis rate, which is worthy of promotion in clinic.
