脊柱高分辨率CT及MRI诊断脊柱转移瘤的对比研究 被引量:10

摘要 目的探讨脊柱高分辨率CT及MRI成像诊断脊柱转移瘤临床应用价值。方法回顾性分析32例经病理及临床证实为脊柱转移瘤患者的脊柱高分辨率CT及MRI成像影像学表现,比较2种检查方法在诊断脊柱转移瘤中的临床应用价值。结果MRI对脊柱转移瘤的检出率为93.0%,明显高于高分辨率CT检出率的80.6%(P<0.05);MRI对于溶骨性骨质破坏、累及附件、病理性压缩骨折及椎旁软组织受累情况的诊断均优于高分辨率CT(均P<0.05);2种检查方法对成骨性骨质破坏、混合性骨质破坏及椎管狭窄诊断比较差异均无统计学意义(均P>0.05)。结论 MRI、高分辨率CT对脊柱转移瘤的诊断各有优势,但MRI在发现早期病变、病理性压缩性骨折、附件及软组织受累等方面较有优势。 Objective To evaluate the clinical application of high-resolution CT and MRI in the diagnosis of spinal metastases.Methods High-resolution CT and MRI findings of 32 patients with pathologically and clinically confirmed spinal metastases were analyzed retrospectively.The clinical values of the two methods in the diagnosis of spinal metastases were compared.Results MRI showed a higher detection rate of spinal metastases than high-resolution CT (93.0% vs 80.6%,P 0.05). Conclusion The MRI and high-resolution CT have their own respective advantages in the diagno-sis of spinal metastases.However,MRI is superior to high-resolution CT for detecting early le-sions,pathologic compression fractures,accessory involvement and paraspinal soft tissue involve-ment.
