气管插管时机对重症社区获得性肺炎患者预后的影响

The effect of timing of tracheal intubation on the prognosis in patients with severe community-acquired pneumonia

目的:探讨影响重症社区获得性肺炎(severe community-acquired pneumonia,SCAP)患者预后的危险因素。方法:回顾性分析2012年1月至2014年6月本地区两所三级医院收治的134例需行气管插管机械通气的SCAP患者资料。结果:影响患者预后的独立危险因素有慢性充血性心力衰竭和气管插管时间(风险比分别为2.18和2.11,P<0.05)。结论:气管插管时间和慢性充血性心力衰竭是影响住院SCAP患者预后的危险因素。

Objective： To explore the risk factors for prognosis of severe community-acquired pneumonia（ SCAP）. Methods： The data of 134 patients with SCAP who needed to be given mechanical ventilation from two local third-class hospitals between January 2012 and June 2014 were retrospectively analyzed. Results： Chronic congestive heart failure and the timing of tracheal intubation were the independent risk factors which had an influence on the prognosis of patients with SCAP（ HR 2. 18 and 2. 11,respectively,P〈0. 05）. Conclusions： Chronic congestive heart failure and the timing of tracheal intubation are the independent risk factors having an effect on the prognosis of in-patients with SCAP.
