2型糖尿病中医证候量化诊断标准的研究

Study on the Standard of Quantitative Diagnosis of Syndromes in Type 2 Diabetes

目的:通过对临床资料的回顾性调查分析和前瞻性研究,探讨2型糖尿病患者的常见证型及证候内涵。方法:采用文献调研法和临床流行病学DME(design measurement evaluation)设计方法对640例和110例病例分别进行回顾性调查分析和前瞻性临床验证,采用因子分析,方差最大化正交旋转和聚类分析相结合的多元统计分析方法进行统计学处理,归纳总结出2型糖尿病的常见证型和证候内涵。结果:2型糖尿病中医证型可以归纳为胃热炽盛型、胃热滞脾型、气阴两虚型、肺热津伤型、阴虚夹瘀型、阴阳两虚型。结论:病证辨证标准研究的思路是在回顾性研究的基础上进行前瞻性临床验证,以验证和修订证候诊断标准,制定2型糖尿病的辨证标准。

Objective： Through reviewing and analyzing the clinical data, to explore the common syndrome type and its conno- tation of patients with type 2 diabetes. Method： Using the literature research method and clinical epidemiology DME （design mea- surement evaluation） design method, the 640 cases and 110 cases were retrospectively analyzed respectively and prospective clini- cal validated. With factor analysis, maximum variance orthogonal rotation and cluster analysis of multivariate statistical analysis method of combining statistical processing, the author summarized the common syndrome type and its connotation. Results： The TCM syndrome types of Type 2 diabetes were summarized as： intense stomach-heat , spleen stomach heat stagnation, deficiency of both QI and YIN, body fluid impairment due to lung heat, Yin deficiency and blood stasis and yin-yang deficiency. Conclusion： The thinking of researching the standard of differentiation is to do prospective clinical validation based on the retrospective study, then to verificate and revise the diagnostic criteria, and develop standards for syndrome differentiation of Type 2 Diabetes.
