磁共振弥散加权成像联合经直肠超声定位经直肠前列腺穿刺活检80例诊断分析 被引量:10

摘要 目的验证磁共振弥散加权成像(MRDWI)对前列腺癌的高敏感性,并对前列腺癌诊断过程中予以MRDWI联合经直肠超声(TRUS)定位经直肠前列腺穿刺活检的效果进行分析,以期提高前列腺癌的检出率。方法选择筛查血清前列腺特异抗原(PSA)升高且均行TRUS和MRDWI检查的患者80例(平均PSA 9.8 ng/m L),经泌尿外科医师、MRI医师、超声医师多学科会诊后确定可疑病灶区域;TRUS引导下经直肠12点前列腺穿刺法加可疑病灶穿刺1-2针;确定病灶中心距B超探头距离,可依TRUS图像实施精确的穿刺到达MRDWI可疑结节。穿刺各针标本注明穿刺部位及是否为MRDWI或TRUS的可疑病灶后分瓶送病理检查。结果一共获取1 020个标本,经诊断一共有212个前列腺癌阳性针数,其中153个为MRDWI阳性,敏感度72.2%(153/212),阳性预测值55.0%(153/278),特异度为84.5%(683/808);B超阳性64个,敏感度为30.2%(64/212),阳性预测值58.7%(64/111),特异度为94.2%(761/808)。一共有102处(48.1%)经B超检查未发现但经MRDWI发现,13处(6.1%)经B超检查发现但经MRDWl检查未发现,联合定位穿刺敏感度78.3%(166/212),与B超定位敏感度比较差异有统计学意义(P〈0.05)。结论 MRDWI对前列腺癌诊断的敏感度优于TRUS;MRDWI与TRUS联合定位进行12针法前列腺穿刺,可以显著减少前列腺癌的漏诊率。
