二次剖宫产与首次剖宫产术中出血对比和预防分析

Comparison and Prevention Analysis of Hemorrhage during Operation of Secondary Cesarean Section and First Cesarean Section

目的：对比二次剖宫产与首次剖宫产术术中出血情况并预防.方法：354例二次剖宫产术产妇为观察组,965例首次剖宫产术产妇为对照组,比较两组产妇术中出血情况,分析其出血原因.结果：观察组术中出血率及出血量均显著高于对照组（P〈0.05）,两组产妇胎盘因素、宫缩乏力比较存在显著性差异（P〈0.05）.结论：二次剖宫产较首次剖宫产更容易出血术中出血,主要原因包括胎盘因素、宫缩乏力,医院应加强管理及护理,提高助产技术水平,降低剖宫产率.

Objective：To compare the intraoperative bleeding and prevention of secondary and first cesarean section. Methods：354 cases of puerpera with secondary cesarean section was in observation group, 965 cases of puerpera with first cesarean section was in control group, situation of intraop-erative bleeding between the two groups were compared, and the reasons of bleeding were analyzed. Results：Intraoperative hemorrhagic rate and amount of bleeding in observation group were both significantly higher than those of control group （P〈0.05）, there was significant difference in pla-cental factors and uterine inertia of the two groups （P〈0.05）. Conclusion：Secondary cesarean section is more likely to bleed during operation than first cesarean section, the main reasons including placental factors and uterine inertia, hospitals should strengthen management and nursing, in order to improve the technical level of midwifery and reduce the rate of cesarean section.
