糖化血红蛋白水平与晚期胃癌FOLFOX方案化疗的近期有效率相关性研究

Association of the Glycosylated Hemoglobin Levels with the Short-term Effect of FOLFOX Chemotherapy in Advanced Gastric Cancer

目的研究晚期胃癌患者不同的糖化血红蛋白水平与化疗的近期有效率的相关性。方法对52例晚期胃癌患者进行回顾性分析,测定化疗前后的患者糖化血红蛋白水平的变化,观察该指标与近期有效率(RR)的相关性。结果全组52例患者均完成至少3个周期化疗,并行疗效评价。该组患者FOLFOX方案化疗的近期有效率为32.7%,晚期胃癌患者化疗后糖化血红蛋白水平与患者不同的病灶数目的差异有统计学意义(P=0.039)。晚期胃癌患者糖化血红蛋白的低、中、高水平组RR分别为9、6、2例,3组患者化疗的近期有效率比较,差异有统计学意义(P<0.05);低、中、高糖化血红蛋白水平组疾病控制率分别为89.4%(17/19)、76.1%(16/21)、41.6%(5/12),组间比较,差异均有统计学意义(P<0.05)。结论糖化血红蛋白水平较高的晚期胃癌患者近期有效率较差,糖化血红蛋白水平可能是潜在的化疗近期疗效预测指标之一。

Objective To explore the correlation of the different glycosylated hemoglobin levels with the short-term effect of chemo-therapy in advanced gastric cancer.Methods Past immunohistochemical data of 52 advanced gastric cancer patients after FOLFOX chemotherapy were analyzed retrospectively.According to the change of the glycosylated hemoglobin levels before and after chemotherapy, it could be observed the association of the glycosylated hemoglobin levels with the short-term effect of chemotherapy.Results The whole group of 52 patients had completed at least three cycles of chemotherapy, and then got completed efficacy evaluation.Recent response rate of the patients who got FOLFOX chemotherapy was 17 cases （32.7%）.The glycosylated hemoglobin levels with the different number of metastasis （ P=0.036） were significantly different.Advanced gastric cancer patients with low, medium and high level expression of gly-cosylated hemoglobin group RR was 9, 6, 2 cases.The three groups of efficient comparative difference was statistically significant （ P〈0.05）.The low, medium and high glycosylated hemoglobin levels of disease control rates were 89.4% （17/19）, 76.1% （16/21）, 41.6%（5/12）.The three groups of efficient comparative difference was statistically significant （P〈0.05）.Conclusion Advanced gastric cancer patients with high glycosylated hemoglobin levels were not sensitive to FOLFOX chemotherapy.The glycosylated hemoglobin levels may be a potential predictor of short-term effect in the chemotherapy.
