右美托咪定复合全身麻醉对老年患者术后早期认知功能的影响 被引量:14

摘要 目的评价右美托咪定复合全身麻醉对老年患者术后早期(7 d内)认知功能的影响。方法择期全麻手术患者120例,性别不限,ASAⅡ或Ⅲ级,年龄65~85岁,体质量55~80 kg。采用随机数字表法将患者分为2组各60例,B组麻醉诱导前15 min经静脉输注右美托咪定负荷剂量0.6μg/kg,随后以0.2μg/(kg·h)的速率持续静脉输注,手术结束前30 min停止;A组给予同等剂量的生理盐水。麻醉诱导:负荷剂量输注完毕5 min后,2组均分别静脉注射舒芬太尼0.2~0.4μg/kg、依托咪酯0.1~0.3 mg/kg和顺阿曲库铵0.15 mg/kg,气管插管后行机械通气。麻醉维持:吸入七氟醚(呼气末靶浓度1%~3%),间断静脉注射舒芬太尼15~25μg,间断静脉注射顺阿曲库铵0.06~0.1mg/kg,维持血流动力学稳定。记录依托咪酯、舒芬太尼和七氟烷用量,苏醒期躁动及术后24 h内谵妄发生情况,并分别于术前1 d及术后1~7 d对患者进行认知功能测验。结果 B组术后7 d内认知功能障碍发生率显著低于A组(P〈0.05),B组麻醉术后1,7 d躁动及谵妄发生率低于A组(P〈0.05)。结论右美托咪定可减少老年患者术后早期认知功能障碍的发生。
