窒息新生儿血清磷酸肌酸激酶同工酶与肌钙蛋白I的变化及其临床意义 被引量:3

摘要 目的探讨窒息新生儿血清磷酸肌酸激酶同工酶(CK-MB)与肌钙蛋白I(c Tn I)的变化及其临床意义。方法选取本院2011年1月-2013年11月的117例窒息新生儿作为观察组,其中轻度窒息组70例,重度窒息组47例。选取同期出生的128例无窒息新生儿作为对照组,比较3组的CK-MB及c Tn I水平。结果轻度窒息组和重度窒息组的CK-MB、c Tn I水平显著高于对照组,差异有统计学意义(P〈0.05)。重度窒息组的CK-MB、c Tn I水平显著高于轻度窒息组,差异有统计学意义(P〈0.05)。4 d窒息新生儿的CK-MB、c Tn I水平显著低于1、2、3 d窒息新生儿,差异有统计学意义(P〈0.05)。3 d窒息新生儿的CK-MB、c Tn I水平显著低于1、2 d窒息新生儿,差异有统计学意义(P〈0.05)。2 d窒息新生儿的CK-MB、c Tn I水平显著低于1 d窒息新生儿,差异有统计学意义(P〈0.05)。结论窒息新生儿的缺氧程度同心肌损害程度成正比,血清CK-MB及c Tn I水平可以作为窒息新生儿心肌损害的诊断指标。 Objective To explore the changes and clinical significance of serum creatine phosphate kinase (CK-MB) and troponin I (cTnI) in asphyxial newborns. Methods 117 asphyxial neonates from January 2011 to November 2013 in our hospital were selected as the observation group.Among them,70 cases were in mild asphyxia and 47 cases were in severe asphyxia.128 neonates without asphyxia in the same period were selected as the control group.The level of CK- MB and eTr-I among three groups was compared. Results The level of CK-MB and cTnI in the severe asphyxia group and mild asphyxia group was higher than that in the control group respectively,with significant difference (P〈0.05).The level of CK-MB and cTnI in the severe asphyxia group was higher than that in the mild asphyxia group,with significant difference (P〈0.05).The level of CK-MB and cTnI in four-day-old children patients was lower than that in one,two and three-day-old children,with significant difference (P〈0.05).The level of CK-MB and cTnI in three-day-old children patients was lower than that in one and two-day-old children,with significant difference (P〈0.05).The level of CK-MB and cTnI in two-day-old children patients was lower than that in one-day-old children,with significant difference (P〈 0.05). Conclusion The degree of hypoxia in asphyxial newborns has a positive correlation with the severity of myocar- dial damage.The level of serum CK-MB and cTnI can be considered as diagnostic indicators of myocardial damage in neonates with asphyxia.
