血府逐瘀胶囊联合长春西汀注射液治疗糖尿病周围神经病变临床研究

Clinical Research of Blood Stasis Expelling Capsules Combined with Vinpocetine Injection for Treating Diabetic Peripheral Neuropathy

目的：观察血府逐瘀胶囊联合长春西汀注射液治疗糖尿病周围神经病变的临床疗效。方法：选取2011年11月—2014年10月就诊于本院的糖尿病周围神经病变患者114例,按照随机数字表法分为对照组和观察组患者各57例。对照组在常规降糖治疗、饮食指导等基础上给予长春西汀注射液20~30 mg静脉滴注治疗;观察组患者则在此基础上给予血府逐瘀胶囊内服治疗,每次6粒,日2次,1周为1个疗程,共治疗4个疗程。治疗后观察两组患者血糖、血脂水平的变化情况,监测患者治疗前后正中神经和腓总神经的感觉传导速度及运动传导速度的变化情况。结果：经治疗后患者的血糖得到控制,血糖水平有所下降,血脂代谢得到改善,总胆固醇（TC）、三酰甘油（TG）、低密度脂蛋白胆固醇（LDL-C）水平较治疗前有所下降,高密度脂蛋白胆固醇（HDL-C）的水平则有所升高,变化幅度以观察组患者更为突出,差异有统计学意义（P〈0.05）;经治疗后两组患者的正中神经和腓总神经的感觉传导速度及运动传导速度较治疗前更快,观察组的速度提高幅度更为明显,差异有统计学意义（P〈0.05）。结论：血府逐瘀胶囊联合长春西汀注射液能显著改善糖尿病周围神经病变患者的血糖水平,调节血脂代谢,利于促进神经功能的恢复,提高神经的传导速度,疗效显著。

Objective： To observe the clinical effects of Blood Stasis Expelling Capsules combined with Vinpocetine Injection on diabetic peripheral neuropathy（ DPN）. Methods： 114 cases with DPN received by this hospital from November,2011 to October,2014 were randomized into observation group and control group,with 57 cases in each group. Control group were given conventional hypoglycemic therapy,dietary instruction and Intravenous dripping of Vinpocetine with 20 ~ 30 mg; observation group were additional given Blood Stasis Expelling Decoction,6 capsules each time and twice every day,one week as a treatment course and for4 treatment courses. The changes of blood sugar and level of blood lipid in both groups were observed; the changes of sensory conduction velocity and motor conduction velocity of the median nerve and nervus peroneus communis after the treatment were detected. Results： After the treatment,the blood sugar got controlled and the level of it got declined,the blood lipid metabolism got improved,the levels of TC,TG,LDL-C got declineded compared these before the treatment,the level of HDL-C increased and the changes was much bigger in observation group; the difference had statistical significance（ P 〈0. 05）. After the treatment,the sensory conduction velocity and motor conduction velocity of the median nerve and nervus peroneus communis became faster than these before the treatment and the improvement was much greater in observation group; the difference had statistical significance（ P 〈0. 05）. Conclusion： Blood Stasis Expelling Capsules combined with Vinpocetine Injection can improve the level of blood sugar in patients with DPN,regulate the blood lipid metabolism,promote recovery of the nervous function,increase the conduction velocity of nerves,which is remarkably effective.
