胰腺癌中COX-2、Her-2/neu的表达及与预后的关系

Correlation between the expression of COX-2 or Her-2/neu and prognosis of pancreatic carcinoma

目的检测COX-2和Her-2/neu在胰腺癌中的表达情况,探讨COX-2和Her-2/neu与胰腺癌临床特征的关系及两者之间的相互关系。方法 50例原发性胰腺癌组织标本和36例癌旁组织标本,应用免疫组化法及实时荧光定量PCR法(RT-QPCR)检测胰腺癌及癌旁胰腺组织中COX-2和Her-2/neu的表达,并计数微血管密度(MVD);分析COX-2和Her-2/neu表达与胰腺癌病理参数的关系,并对二者之间的相关性进行研究。结果胰腺癌组织均过表达COX-2和Her-2/neu;COX-2高表达与胰腺癌增殖活性、TNM临床分期及肿瘤浸润转移等临床病理特征相关(P<0.05),而与年龄、性别、发病部位、病理组织分级无关;Her-2/neu高表达与胰腺癌组织分级、TNM临床分期及肿瘤浸润转移关系密切(P<0.05),与胰腺癌患者的年龄、性别、发病部位、增殖活性无关;COX-2、Her-2/neu在胰腺癌中的表达呈正相关,且二者均和MVD呈正相关(P<0.05)。预后≤1的患者COX-2、Her-2/neu阳性表达率显著低于预后>1年的患者(P<0.05)。结论 COX-2和Her-2/neu在胰腺癌中的表达呈正相关关系,联合检测二者的表达对胰腺癌的诊断、治疗和预后有一定的意义。

Objective To investigate the expression of COX-2 or Her-2 /neu in pancreatic carcinoma,and to explore the correlation between their expressions and clinical characteristics of pancreatic carcinoma as well as the relationship between them.Methods The expression levels of COX-2 and Her-2 / neu were detected by immunohistochemistry（ RT-QPCR） in 50 cases of primary pancreatic carcinoma and 36 cases of tumor adjacent tissues,and microvessel density（ MVD） was calculated.The correlation between their expressions and clinical pathological parameters of pancreatic carcinoma as well as the relationship between the two factors were analyzed.Results The overexpression of COX-2 and Her-2 / neu appeared in pancreatic carcinoma tissues,furthermore,the overexpression of COX-2 was closely correlated to the proliferative activity,infiltration and metastasis,TNM clinical staging,one-year survival rate of pancreatic carcinoma（ P〈0.05）,however,which was not related with patient＇s age,sex,anatomical site and histological grade of pancreatic carcinoma.The high-expression of Her-2 / neu was closely correlated to histological grade,TNM clinical staging and infiltration and metastasis of pancreatic carcinoma（ P〈0.05）,however,which was not related with patient＇s age,sex,anatomical site and proliferative activity of pancreatic carcinoma.The expression of COX-2 was positively correlated to that of Her-2 / neu in pancreatic carcinoma,moreover,both of them were positively related with MVD（ P〈0.05）.The positive expression rates of COX-2 and Her-2 / neu in patients with prognosis ≤1 year were significantly lower than those of patients with prognosis 〉1 year（ P〈0.05）.Conclusion The expression of COX-2 is positively correlated to that of Her-2 / neu in pancreatic carcinoma,and combination detection of COX-2 with Her-2 / neu may play a certain role in the diagnosis,treatment and prognosis of pancreatic carcinoma.
