健脾清化方对慢性乙型肝炎脾虚湿盛证患者T细胞亚群的影响

Effect of Jianpi Qinghua Decoction On Chronic Hepatitis B Patients With Dampness T Cell Subsets

目的:探讨健脾清化方对慢性乙型肝炎(chronic hepatitis B,CHB)脾虚湿盛证患者细胞免疫功能的影响。方法:将153例中医辨证属脾虚湿盛证的CHB患者随机分为治疗组和对照组,其中治疗组79例,对照组74例。对照组给予重组人干扰素α-2b针抗病毒治疗,治疗组在对照组基础上加服健脾清化方治疗,每日1剂,每日2次。两组疗程均为6个月。结果:治疗前后比较,两组在对患者纳差或食后胃脘胀满、便溏或黏滞不畅、腹胀、气短乏力等症状,以及对丙氨酸转氨酶、天冬氨酸转氨酶的改善均有一定的效果,且治疗组在改善上述症状及丙氨酸转氨酶、天冬氨酸转氨酶方面明显优于对照组(P<0.05);在降低乙型肝炎病毒核糖核酸方面,治疗组较对照组疗效显著(P<0.05);治疗组在治疗后CD3+、CD4+均升高,CD8+下降,CD4+/CD8+比值升高,与对照组比较,差异有统计学意义(P<0.05)。结论:健脾清化方可改善CHB脾虚湿盛证患者细胞免疫紊乱状态,提高干扰素抗病毒的临床疗效。

Objective：Investigating the effect of Jianpi Qinghua Decoction on the cellular immune function of patients with CHB spleen deficiency and wet overflow syndrome.Methods：1 53 cases with spleen dampness syndrome of CHB patients were randomly divided into treatment group and control group,including 79 cases in the treatment group,74 cases of control group.Recombinant human interferon alpha 2b needle antiviral therapy was given to the control group and the treatment group on the basis of Jianpi Qinghua Decoction,daily 1 agent,2 times a day.Two groups of treatment were 6 months.Results：Before and after the treatment,The anorexia or postprandial epi-gastric fullness,loose stools or viscous poor,abdominal distension,shortness of breath,fatigue and other symptoms,and the alanine amin-otransferase（ALT）,aspartic acid transaminase of the two groups were improved in a certain effect,and the treatment group in improving the symptoms,and alanine aminotransferase （ALT）,aspartic acid transaminase was significantly better than the control group （P ＆lt;0.05）;in reducing hepatitis B virus ribonucleic acid（RNA）,the treatment group was significantly more effective than the control group （P ＆lt;0.05）;the CD3 ＋,CD4 ＋ of the treatment group were increased after treatment,CD8 ＋ decreased,CD4 ＋/CD8 ＋ ratio increased, compared with the control group,the difference was statistically significant（P ＆lt;0.05）.Conclusion：Jianpi Qinghua Decoction can improve the cellular immune dysfunction ofthe patients with CHB spleen deficiency and wet overflow syndrom and improve the clinical effect ofin-terferon virus.
