乌司他丁联合生长抑素对重症急性胰腺炎大鼠血清炎性细胞因子的影响 被引量:25

摘要 目的研究乌司他丁联合生长抑素用药对重症急性胰腺炎(SAP)炎性细胞因子的影响。方法建立大鼠急性胰腺炎模型成功后,将大鼠分为4组:重症急性胰腺炎(SAP)组、乌司他丁(U)组、生长抑素(S)组、乌司他丁组联合生长抑素(U+S)组;另加假手术(SO)组,每组各10只。各组大鼠于造模后12 h下腔静脉取血检测血清淀粉酶(AMY)活性、肿瘤坏死因子-α(TNF-α)、细胞间粘附分子(ICAM-1)、白介素-1β(IL-1β)水平,观察胰腺病理学改变并进行胰腺病理学评分。结果 1血清AMY、TNF-α、ICAM-1及IL-1β水平:与SO组相比,其余4组AMY、TNF-α、ICAM-1及IL-1β水平均升高(P<0.05);与SAP组相比,乌司他丁、生长抑素单给药组AMY、TNF-α、ICAM-1及IL-1β水平均降低,与单给药组相比,乌司他丁组联合生长抑素组AMY、TNF-α、ICAM-1及IL-1β水平进一步降低(P<0.05)。2胰腺组织病理学表现:与假手术组相比,其余4组胰腺组织损伤严重;与SAP组相比,单给药组及乌司他丁联合生长抑素组病理损伤减轻;与单给药组相比,乌司他丁联合生长抑素组病理损伤进一步减轻。3胰腺病理学评分:与SO组相比,其余4组胰腺病理学评分均升高(P<0.05);与SAP组相比,单给药组乌司他丁组、生长抑素组胰腺病理学评分均降低,与单给药组相比,乌司他丁组联合生长抑素组胰腺病理学评分进一步降低(P<0.05)。结论乌司他丁联合生长抑素能够有效减轻SAP大鼠胰腺损伤,其保护机制可能与降低炎性因子的表达有关。 Objective To detect the effects of ulinastatin combined with somatostatin on serum inflammatory cytokines of severe acute pancreatitis( SAP) in rats. Methods To construct severe acute pancreatitis( SAP) in rats. After the successful finish model,the severe acute pancreatitis group was divided randomly into 5 groups: severe acute pancreatitis group( SAP group),ulinastatin group( U group),somatostatin( S group),ulinastatin combined with somatostatin group( U + S group). And add to sham operation group( sham group),10 rats were in each group. Serum level of amylase( AMY),tumor necrosis factor alpha( TNF- α),intercellular adhesion molecule( ICAM- 1) and interleukin- 1 beta( IL- 1β) were detected through the inferior vena at 12 hours after the model setting up. Pathologic change of the pancreas was observed under light microscope and the pancreatic pathology score was performed. Results Compared with the sham groups,the serum levels of AMY,TNF- α,ICAM- 1 and IL- 1β increased significantly in the other four groups( P〈0. 05). Compared with the SAP groups,the serum levels of AMY,TNF- α,ICAM- 1 and IL- 1β decreased in the single administration groups( P〈0. 05). Compared with the single administration groups,the serum levels of AMY,TNF- α,ICAM- 1 and IL- 1β decreased in the combined administration group( P〈0. 05). Compared with the sham group,the damage of the pancreas aggravated in the other four groups( P〈0. 05). Compared with the SAP group,the pathology of the pancreas tissues ameliorated compared with single administration groups. Compared with single administration groups,the pathology of the pancreas tissues further ameliorated compared with the combined administration group. Compared with the sham groups,the pathology scores were increased significantly in the other four groups( P〈0. 05). Compared with the SAP groups,the pathology scores were decreased in the single administration groups( P〈0. 05). Compared with the single administration groups,the pathology scores were decreased in the combined administration group( P〈0. 05). Conclusion Ulinastatin combined with somatostatin can effectively reduce the pancreatic damage in SAP rats,which the underlying mechanism may be related to the reduction of inflammatory factors.
