康复运动对老年心绞痛患者心脏事件及脂代谢的影响

Influence of rehabilitation exercise for elderly patients with angina pectoris on cardiac events and lipid metabolism

目的观察康复运动对老年心绞痛患者心脏事件及脂代谢的影响。方法将确诊为稳定性心绞痛的老年患者256例随机分为治疗组(128例)和对照组(128例),两组患者均采用常规疗法。治疗组还接受不同阶段、不同运动强度的运动训练。随访时间持续1 a,对随访期间内两组患者的心脏事件(如不稳定型心绞痛、急性心肌梗死、冠脉介入治疗、心源性猝死等)发生情况进行统计,并对比两组患者治疗前后血脂水平的变化情况。结果治疗组不稳定性心绞痛发生率下降60%,急性心肌梗死发生率下降67%,需进行冠脉介入术的发生率减少50%,其总体冠心病事件危险性下降60%,两组患者总胆固醇、低密度脂蛋白均较入选时明显改善,以治疗组患者总胆固醇、低密度脂蛋白、甘油三酯、高密度脂蛋白的改善幅度较显著,与对照组间差异均具有统计学意义(P<0.05)。结论康复运动能明显降低老年心绞痛患者心脏事件的发生率,改善脂代谢。

Objective To observe the influence of rehabilitation exercise for elderly patients with angina pectoris on cardiac e-vents and lipid metabolism.Methods 256 elderly patients who were diagnosed as angina pectoris were randomized into two groups as the treatment group （128 cases）and the control group （128 cases）.Both of the two groups underwent routine treatment,the treatment group accepted different stages and intensity exercise in addition.All the patients were followed up for one year.Cardiac events （such as unstable angina pectoris,acute myocardial infarction,coronary artery interventional therapy and sudden cardiac death）of both groups were recorded,and the lipid level of both groups before and after treatment were tested.Results Compared with the control group,the incidence of unstable angina pectoris,acute myocardial infarction,and coronary artery interventional therapy in the treatment group decreased by 60%,67% and 50% respectively.The overall risks of coronary heart events decreased 60%.The total cholesterol and low density lipoprotein cholesterol of both groups were ameliorated compared with the baseline level. In the treatment group,the extent of total cholesterol,low density lipoprotein cholesterol,triglyceride,high density lipoprotein cho-lesterol were ameliorated significantly comparing with the control group （P 〈0.05）.Conclusion Rehabilitation exercise could re-duce the cardiac events of senile angina pectoris and ameliorate their lipid metabolism simultaneously.
